肝硬化腹水患者血清-腹水白蛋白梯度与食管静脉曲张破裂出血及肝功能分级的关系研究

摘　　要：目的探讨肝硬化腹水患者血清一腹水白蛋白梯度与食管静脉曲张破裂出血及肝功能分级的关系。方法选择商丘市第三人民医院在2010年5月至2013年7月收治的肝硬化腹水患者116例，所有患者均通过胃镜检查食管静脉曲张破裂出血情况，并根据检查结果分为食管静脉曲张破裂出血组（59例）和非出血组（57例）。分别采集患者空腹静脉血计算出血清一腹水白蛋白梯度（SAAG）值，并比较分析不同Child分级的患者其SAAG值的差异。结果两组患者血清一腹水白蛋白水平均〉11g／L，食管静脉曲张破裂出血组患者SAAG水平显著高于非出血组，两组比较差异有统计学意义（t=14．8524，P〈0．01）；同时，不同SAAG水平的患者其静脉曲张破裂出血及肝功能分级情况比较差异有统计学意义（P〈0．05），随着SAAG水平的增高，食管静脉曲张出血及肝功能受损程度越严重，组间比较差异有统计学意义（P〈0．01）。结论血清一腹水白蛋白梯度与食管静脉曲张破裂出血及肝功能分级关系密切，对临床肝硬化腹水的预测及诊断具有一定的临床价值。Objective To investigate the correlation among serum-ascites album in gradient, esophageal variceal bleeding and Child-Pugh in patients with liver cirrhosis ascites. Methods One hundred and sixteen patients with liver cir- rhosis ascites received medical treatment in the third people＇ s hospital of Shangqiu from May 2010 to July 2013 were given gastroscopy esophageal variceal bleeding, and according to the cheek results, all patients were divided into bleeding group （59 cases） and non bleeding group （57 cases）. Then the fasting venous blood were collected for determining the serum-as- cites album in gradient of two groups and compared among different grades of liver function impairment determined by the Child-Pugh＇ s score. Results The serum-ascites album in gradient of the two groups were greater than 11 g/L, the SAAG value of bleeding group was significantly higher than that of non bleeding group, and the difference was statistical significant （ t = 14. 8524, P 〈 0. 01 ） ; simultaneously, the esophageal variceal bleeding and Child-Pugh were significantly different in patients with different serum-ascites album in gradient, the esophageal variceal bleeding and Child-Pugh associated with SAAG level （P 〈 0. 01 ）. Conclusions There is close correlation of esophageal variceal bleeding with liver function dam- age, therefore it has certain clinical application value to predict patients with liver cirrhosis ascites.
