健脾补肾复方联合化疗治疗大肠癌患者疗效指标的系统评价

摘　　要：目的:系统评价化疗联合健脾补肾中药与单纯化疗治疗比较大肠癌患者疗效、生活质量及生存率。方法:检索中国生物医学文献光盘数据库、中国期刊全文数据库、万方数据医药信息系统、PUBMED、MEDLINE、EMBASE,选择1990—2014年4月公开发表的有关健脾补肾方药联合西药化疗治疗大肠癌的相关文献,运用Cochrane协作网提供的Rev Man5.1软件分析,以OR值作为效应指标,主要对实体瘤疗效总有效率(CR+PR)、实体瘤疗效稳定率(CR+PR+NC/CR+PR+NC+PD)、KPS评分、生存率进行观察,计算中西医结合治疗对单纯化疗治疗的优势。结果:治疗组瘤体疗效总有效率(CR+PR)优于对照组,差异有统计学意义[OR=1.89,95%CI=1.14-3.14,P<0.05];治疗组瘤体疗效稳定率优于治疗组,差异有统计学意义[OR=2.09,95%CI=1.07-4.09,P<0.05];治疗组KPS评分优于对照组,差异有统计学意义[OR=5.90,95%CI=2.10-16.55,P<0.05];治疗组1、3、5年生存率均优于对照组,差异有统计学意义[OR=3.12、2.09、1.87,95%CI=1.13-8.64、1.14-3.80、1.05-3.34,P<0.05]。结论:中西医结合治疗大肠癌的疗效优于单纯西药,但纳入研究质量均为低质量,导致发表存在偏倚,需要高质量研究做进一步验证。Objective： To assess efficacy,quality of life and survival rate of the colorectal cancer patients receiving Jianpibushen Compound Recipe combined with chemotherapy. Methods： By searching CBMdisc,CJFD,Wangfang Data,PUBMED,MEDLINE,EMBASE. The search was updated to April 2014. Software Revman5. 1,provided by Cochrane Library,was used to perform systematic review method. The combined odds ratio（ OR） and its 95% confidence interval（ CI） were calculated as well. Results： Five RCTs and seven CCTs were analyzed. The results showed that Jianpibushen Compound Recipe combined with chemotherapy was better than chemotherapy in CR ＋ PR（ OR = 1. 89,95% CI = 1. 14-3. 14,P〈0. 05）,CR ＋ PR ＋ NC / CR ＋ PR ＋ NC ＋ PD（ OR =2. 09,95% CI = 1. 07-4. 09,P〈0. 05）,KPS score（ OR = 5. 90,95% CI = 2. 10-16. 55,P〈0. 05） and five years＇ survival rate（ OR = 3. 12,2. 09,1. 87,95% CI = 1. 13-8. 64,1. 14-3. 80,1. 05-3. 34,P〈0. 05）. Conclusion： Jianpibushen Compound Recipe combined with chemotherapy is effective for colorectal cancer,and may be better for only chemotherapy. However,the quality of the RCTs and CCTs using Jianpibushen Compound Recipe combined with chemotherapy on colorectal cancer should be improved.
