3.0T磁共振扩散加权成像在评估宫颈癌化疗疗效中的应用价值

摘　　要：目的:探讨3.0T磁共振的扩散加权成像技术在宫颈癌化疗前后效果评估方面的价值.方法:对29例接受放化疗的中晚期宫颈癌患者,于治疗前和治疗开始后1个月分别行常规MRI平扫及DWI检查.测量治疗前和治疗后T2WI上肿瘤最大径和ADC值,疗效依据治疗前后肿瘤最大径变化率分为:肿瘤消失(CR)、肿瘤部分消失(PR)、肿瘤稳定(SD)、肿瘤进展(PD).分析各疗效组治疗前后ADC值间差异、疗效组间治疗前ADC值差异及治疗前ADC值与疗效的相关性.结果:PR组治疗前后ADC值差别具有统计学意义(P<0.05);SD组治疗前后ADC值差别具有统计学意义(P<0.05);PR组治疗前平均ADC值低于SD组(P<0.05);治疗后肿瘤最大径缩小率与治疗前ADC值负相关(r=-0.574,P<0.05).结论:DWI序列有助于监测和评价宫颈癌化疗效果.AIM： To investigate the value of diffusionweighted imaging （DWI） in evaluating the response of uterine cervix canc er to chemotherapy3.0 T MRI. METHODS： A total of 29 pa tients with uterine cervix cancer who underwent chemotherapy re ceived conventional MRI and DWI examination before and at one month after the therapy. The maximum diameter of tumor and ADC value before and after treatment were measured on axi alT2WI. According to changes of tumor diameter over the treat ment, patients were divided into groups： CR （complete re sponse）, PR （partial response）, SD （stable disease） and PD （progressive disease）. The differences between ADC values be fore and after treatment in each group and the mean ADC value of pretreatment among groups were compared. The relation between the pretreatment ADC value and therapeutic response was evalua ted. RESULTS： The differences between ADC values preand posttreatment of PR group were statistically significant （P 〈 0.05）. The differences between ADC preandposttreatment of SD group were statistically significant （ P 〈 0.05 ）. The mean ADC value before therapy of PR group was statistically lower than that of SD group （ P 〈 0.05 ）. Negative corelation between the pre treatment ADC value and therapeutic response was observed （ r = 0.574, P 〈 0.05 ）. CONCLUSION ： DWI can monitor and e valuate the response of uterine cervix cancer to chemotherapy.
