摘 要:目的探讨累及黄斑区的孔源性视网膜脱离(RRD)患眼视网膜黄斑中心凹厚度与多焦视网膜电图(mf-ERG)中央反应波的相关洼。方法临床病例系列研究。对2012年3—12月在上海交通大学附属第六人民医院南院眼科累及黄斑区的RRD16例(16只眼)行巩膜外加压术视网膜复位的患者。手术前与手术后1、3、6个月行频域OCT及mf-ERG检测,观察RRD患者手术前后黄斑区结构与视功能的变化,分析黄斑中心凹厚度与中央反应波振幅密度及潜伏期的相关性。结果手术后1、3、6个月RRD患者黄斑中心凹厚度较术前明显降低差异均有统计学意义(t=10.112、18.794、23.234,均P=0.05);1环N1波和P1波振幅密度较手术前明显提高(t=-27.28、-92.992,-29.794、-94.942,-61.416、-88.672;均P=O.05),潜伏期较手术前缩短(t=32.280、22.262,17.820、23.858,18.947、8.063;均P=0.05),差异均有统计学意义。手术后1、3、6个月患者黄斑中心凹厚度与1环N1波和P1波振幅密度差异有统计学意义(r=-0.534、-0.667,P=0.033、0.005;r=-0.611、-0.679,P=0.012、0.004;-0.500、-0.668,P=0.049、0.005),与潜伏期差异无统计学意义(r=0.255、0.027,P=0.340、0.922;r=0.300、0.058,P=0.258、0.876;r=0.402、0.458,P=0.123、0.074)。结论OCT和mf-ERG联合应用能全面准确地评估视网膜脱离患者黄斑区结构和功能。Objective To explore the correlation between central foveal thickness and central re- action relevance waves with multifocal electroretinogram (mf-ERG) in patients with rhegmatogenous retinal detachment (RRD) involving the macular area. Methods Clinical case series study of 16 pa- tients (16 eyes) with RRD involving the macular area underwent scleral buckling surgery, mf-ERG and OCT examination were performed before and 1, 3, 6 months after surgery. The changes of the structure and visual function in the macular area were observed in RRD patients before and after sur- gery. The correlations between macular foveal thickness and central reaction relevance waves were an- alyzed. Results Compared with before surgery, postoperative 1, 3, 6 months, the foveal retinal thick- ness after surgery decreased significantly, the differences were statistically significant (t =10.112, 18.794, 23.234, all P =0.05); N1 and P1 wave amplitude density at ring 1 improved (t =-27.28,-92.992,-29.794,-94.942,-61.416,-88.672; all P =0.05), the incubation period reduced than before surgery (t =32.280, 22.262, 17.820, 23.858, 18.947, 8.063; all P =0.05), the differences were statistically significant. Postoperative 1, 3, 6 months, N1 and P1 wave amplitude density at ring 1 with central foveal thickness showed negative correlation (r =-0.534, -0.667, P =0.033, 0.005;r =-0.611,-0.679, P =0.012, 0.004; r =-0.500,-0.668, P =0.049, 0.005), and there was no correla- tion between central foveal thickness and the incubation period (r=0.255, 0.027, P =0.340, 0.922; r =0.300, 0.058, P =0.258, 0.876; r =0.402, 0.458, P =0.123, 0.074). Conclusions OCT and mfERG binding applications can assess the structure and function in the macular area in patients with retinal detachment comprehensively and accurately.
