老年性重度上睑下垂手术疗效观察

摘　　要：目的探讨老年性皮肤松弛伴重度上睑下垂的手术方法和临床效果。方法对2011年2月至2014年6月在深圳市眼科医院收治的15例（29只眼）老年性皮肤松弛伴重度腱膜性上睑下垂采用提上睑肌腱膜折叠并前徙联合眼睑部分皮肤切除治疗。术前检查双眼上睑皮肤松弛及上睑下垂程度、提上睑肌肌力、双眼平视睑裂高度等。术中采用切开法重睑成形术设计切口，切除多余松垂的皮肤和眼轮匝肌，联合提上睑肌腱膜折叠并前徙术。术后观察双眼对称性，平视睑裂高度、双重睑弧度等指标。结果15例（29只眼）患者术后双重睑弧度流畅自然，上睑凹陷得到矫正，双眼睑裂高度相差不超过1mm。无眼睑畸形发生，无过矫欠矫，眼睑闭合正常。所有患者随访3个月至2年，美容效果满意。结论提上睑肌腱膜折叠并前徙术联合眼睑部分皮肤切除术治疗老年性眼睑松弛伴重度腱膜性上睑下垂的手术方法疗效确切可靠。手术创伤小，术式简单，手术时间短，能有效改善患者的视觉功能及外观。Objective To evaluate the effects of surgical treatment for senile blepharochalasis with severe ptosis. Methods Fifteen patients （29 eyes） with senile blepharochalasis concomitant se- vere ptosis were treated with levator plication and advancement combined with excision of the redun- dant skin and orbicular muscle. Preoperative ophthalmic examination include the degree of blepharo- chalasis, the degree of ptosis, levator function and palpebral fissure height. The incision was de- signed as the traditional double eyelid operation, the redundant eyelid skin was removed, and com- bined levator plication and advancement. The symmetry of the eyes, palpebral fissure height, the eye- lid crease and contour were observed after operation. Results The eyelid crease and contour was found to be good and natural on 15 patients （29 eyes） postoperatively, upper eyelid sag was correct- ed. There was difference less than 1 mm in the palpebral fissure height of eyes. All patients were followed up for 3 months to 2 years, and the result of combined surgery for senile blepharochalasis with severe ptosis was satisfactory. No complications were reported. Conclusions This one stage surgery is a simple and effective treatment for blepharochalasis with severe ptosis, each other make up the lack of a single operation, shorten operation time, and improved the patient＇s appearance and visual function.
