摘 要:目的探讨改良片段弓矫治技术联合横腭杆拉上颌尖牙向远中的临床效果和特点。方法选择各类需要拔除上颌第一前磨牙或4颗第一前磨牙且需要强支抗的正畸患者24例,上颌放置横腭杆,用改良片段弓先拉上颌尖牙向远中移动,矫治前和移动6个月时拍摄头颅侧位定位片和取上、下颌模型,对测量结果进行统计分析。结果上颌尖牙到RL垂直距离减小约4.34 mm,上颌第一磨牙到SL垂直距离增大约1.19 mm,上颌尖牙后移和上颌磨牙前移距离之比为3.6∶1.0;SN-MP角变化无明显差异。牙弓周长减小3.51 mm,上切牙拥挤度明显减轻。结论改良片段弓联合横腭杆能够高效远中整体移动上颌尖牙,避免了摩擦力的不利影响,且对SN-MP角无明显影响;支抗磨牙近中移动明显小于尖牙远中移动,有效保护了支抗。Objective To investigate the clinical effects and technical characteristics of distalization of upper cuspid by modified seg- mented archwire technique combined with transpalatal arch. Methods 24 patients whose maxillary first premolar should be extracted or whose 4 premolars should be extracted along with the need of maximum anchorage were selected in the study. Transpalatal arches were placed in the upper mandibles,and modified segmented archwires were used for the distalization of upper cuspids. Before treatment and in the 6th month during the treatment,cephalograms were taken, and maxillary and mandibular casts were made. Results were ana- lyzed statistically. Results U3-SL decreased by 4.34 mm and U6s-SL increased by 1.19 mm. The ratio of distance of distalization of up- per cuspid to mesiolization of upper molar was 3.6: 1.0. No significant difference in SN-MP was found. The arch perimeter decreased by 3.51 mm and the crowding of upper incisors was alleviated obviously. Conclusions The distalization of upper cuspid could be effec- tively accomplished by the modified segmented archwire technoque combined with the transpalatal arch,which not only avoided the in- tervention of friction, but also had no adverse effects on SN-MP. Besides, the mesialization of anchorage molar was obviously smaller than the distalization of cuspid,which well protected the anchorage.
