摘 要:目的探讨不同去腐方式、粘结系统及充填材料分别组合应用于龋齿充填治疗,对充填体边缘密闭性及操作复杂程度的影响,以寻求适合偏远地区的龋齿治疗方法.方法选取面有龋、其余牙面无龋的离体牙294颗,随机平均分为ART手工去腐和机械去腐2大组,每组根据不同的粘接系统、充填材料各分为7种组合.体外模拟龋齿充填治疗,记录样本去腐及充填时间,温度循环法模拟体内状况处理样本后,扫描电镜观察,并进行染色渗透实验,单盲法对二维微渗漏情况进行观察,并对结果进行统计学分析.结果 (1)在其余条件相同下,ART手工去腐组耗时明显长于机械去腐组(P<0.05);(2)每大组中,玻璃离子组耗时最短,Easy one+3MF2000/3MZ350次之,再次XenoⅢ+3MF2000/3MZ350,时间最长Single Bond2+3MF2000/3MZ350,选择不同的粘结系统和不同材料充填时间差异有统计学意义(P<0.05);(3)扫描电镜结果:粘结系统与充填材料相同时,ART组与机械去腐组差异无统计学意义(P>0.05);同一去腐方式下,玻璃离子组充填材料与牙体组织间的缝隙最大,XenoⅢ+3MF2000/3MZ350组次之,Easy one+3MF2000/3MZ350组与Single Bond2+3MF2000/3MZ350最小.(4)二维微渗漏结果:粘结系统和充填材料相同的前提下,ART组与机械去腐组差异无统计学意义(P>0.05);Easy one+3MF2000/3MZ350组与Single Bond2+3MF2000/3MZ350组微渗漏最轻,且两组间无显著性差异(P>0.05),其余组间差异有统计学意义(P<0.05).结论使用3MF2000复合体并结合自酸蚀粘结系统Easy One运用到ART技术中,可为偏远地区的龋齿治疗提供一种有效的方法,但远期临床效果有待于进一步研究.Objective In the study,we investigated the various combinations of different methods of removing decayed tooth combined with different adhesive systems and filling materials used in caries treatment, and observed the influence of the marginal- tightness and the complexity of operation, in order to find out a suitable treatment of dental caries for remote area. Methods 294 teeth with only occlusal caries were randomly and equally divided into 2groups, one of which was treated with atraumatic restorative treatment(ART) and the other was treated with the traditional rotating instruments treatment. Then, according to the different adhesive systems and filling materials,each group was divided into 7 sub- groups. In vitro caries filling treatment was applied,and the cavies removing time and filling time were recorded. After simulating the process in mouth by temperature- cycle method, the samples were observed by SEM and the dye penetration experiment. The two- dimensional microleakage was observed by single blind trial, and the results were analyzed using SPSS 12.0 software. Results(1) Using the same adhensive and filling system, caries removing time in ART was longer than the traditional rotating instruments treatment(P〈0.05);(2) In each group, the time in GIC was the shortest, the time of Easy one+3M F2000/3M Z350 group was shorter than that of XenoⅢ+3MF2000/3MZ350, and the time of Single Bond 2+3MF2000/3MZ350 group was the longest. There was significant difference in the filling time with different adhesive systems and filling materials(P〈0.05).(3) SEM results showed: when using the same adhesion agent and the same filling material,there was no significant difference in SEM between ART group and the traditional rotating instruments treatment group(P〉0.05). In the same case of removing decayed tooth, the gaps between the filling material and tooth tissue in group GIC was the biggest, and the XenoⅢ +3MF2000/3MZ350 was the second, while the Easy one +3MF2000/3M Z350 an
