摘 要:目的探讨CD117和DOG-1在胃肠道间质瘤(gastrointestinal stromal tumor,GIST)的表达及意义。方法收集84例GIST患者临床和病理资料,观察其形态学特征,采用免疫组织化学法检测CD117和DOG-1的表达。免疫组织化学检测以肿瘤旁平滑肌组织和神经组织为正常对照。结果 GIST按形态学特征主要分为梭形细胞型(58例)、上皮样型(10例)和混合型(16例),上述形态学类型中CD117和DOG-1表达差异均无统计学意义(均P>0.05)。按生物学行为分为极低(15例)、低度(40例)、中等(18例)和高度(11例)危险程度。CD117和DOG-1在极低和低度危险程度GIST中表达率分别为94.5%和92.7%,在中度和高度危险程度GIST中CD117和DOG-1表达率分别为96.6%和100.0%。在所有84例中联合检测DOG-1和CD117的表达率均为100.0%。肿瘤旁平滑肌组织和神经组织均不表达CD117和DOG-1。DOG-1和CD117的表达与肿瘤病理类型、部位和临床分期均无相关性(均P>0.05)。结论结合临床病理特征[胃肠道原发性软组织肿瘤,梭形和(或)上皮样细胞为主],免疫组织化学联合检测CD117和DOG-1可作为GIST辅助诊断的一线标志物。Objective To investigate the expressions of DOG-1 and CD117 in gastrointestinal stromal tumors( GISTs) and its clinicopathological significance. Methods Eighty-four cases of GISTs were retrieved from pathology database and the clinicopathological characteristics were analyzed. Immunohistochemical method was used to detect the expression of DOG-1 and CD117. Samples of peritumoral smooth muscle and nerve tissue were used as normal control.Results Eighty-four cases of GISTs were classified as spindle cell type( n = 58),epitheloid type( n = 10) and mixed type( n = 16),and there was no significant difference in the expressions of CD117 and DOG-1 among different pathological types( P 0. 05). All cases were graded as very low risk( n = 15),low risk( n = 40),intermediate risk( n = 18) and high risk( n = 11) according to biological behavior. The expression rates of DOG-1 and CD117 in very low and low risk GISTs were94. 5% and 92. 7%,respectively; and those in moderate and high risk GISTs were 96. 6% and 100. 0%,respectively.With combined detection of DOG-1 and CD117 the expression rate was 100. 0%. No samples of peritumoral smooth muscle or nerve tissue expressed DOG-1 and CD117. The expression of DOG-1 and CD117 was not correlated with pathological types,sites and clinical stages. Conclusion In combination with clinicopathological characteristics( primary soft tissue tumors of gastrointestinal tract predominantly composed of spindle-shaped and / or epithelioid cells),immunohistochemical detection of DOG-1 and CD117 may be used as first-line markers for the diagnosis of GISTs.
