急性脑梗死患者血清尿酸水平与颈动脉粥样硬化的关系研究

摘　　要：目的探讨脑梗死患者血清尿酸(uric acid,UA)水平与颈动脉粥样硬化(Carotid artery atherosclerosis,CAA)斑块及脑梗死的关系。方法检测78例脑梗死患者的血清尿酸(UA)、总胆固醇(total cholesterol,TC)、甘油三酯(Triglycerides,TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(Low-density lipoprotein,LDL-C)水平,并应用彩色多普勒超声检测其颈动脉内中膜厚度(intimal-medial wall thickness,IMT)及斑块。结果脑梗死组患者血清UA水平及IMT值高于对照组(P<0.05),脑梗死组内高尿酸(HUA)组TG、LDL水平较尿酸正常组显著增加(P<0.05)。脑梗死组内颈动脉粥样硬化亚组尿酸水平比较,IMT增厚组、斑块形成组尿酸水平较IMT正常组高(P<0.05)。结论 HUA血症可能是脑梗死重要危险因素,且与颈动脉粥样硬化形成有关。Objective To explore the serum uric acid（UA） level in patients with cerebral infarction（CI） and discuss its correlation with carotid artery atherosclerosis （CAA）. Methods Total 78 inpatients with CI were chosen as the cerebral infarction group and 78 inpatients without cerebro-vascular diseases during the same period were chosen as the control group. The serum UA level, the serum total cholesterol（TC）.Triglyc- erides （TG）.Low-density lipoprotein（LDL） and High-density lipoprotein（HDL） were detected, and the inti- mal-medial wall thickness（IMT） and detection of carotid artery plaque were examined by color Doppler ultra- sound. All the datesdata were compared between the two groups. Results The average UA levels and average IMT in cerebral infarction group were higher than those in control group （P〈0. 05）, and the TG and LDL lev- els within the High uric acid subgroup in cerebral infarction group were Ssignificantly higher than those inwith normal uric acid levels in cerebral infarction group （P〈0. 05）. Comparison of serum uric acid levels within the CAA subgroup in cerebral infarction group, the UA levels in IMT thickening subgroup and plaque formation subgroup were higher than those in IMT normal subgroup. Conclusions The high elevation of serum UA level is a main risk factor for cerebral infarction and is closely related to CAA.
