E-cadherin和MMP-2在大肠癌中的表达及其临床意义

摘　　要：目的探讨E-钙粘蛋白(E-cadherin)和基质金属蛋白酶2(MMP-2)的蛋白表达与大肠癌的关系。方法应用免疫组织化学技术检测E-cadherin和MMP-2蛋白在14例大肠腺瘤组织、102例大肠癌组织及相应癌旁正常粘膜的表达情况,分析二者表达与大肠癌发生发展的关系以及二者在大肠癌表达的相关性。结果 E-cadherin在大肠癌旁的正常组织中阳性率为68.6%,在癌组织中阳性率为42.2%,表达明显降低(P<0.01);相反MMP-2蛋白在癌组织中阳性率为50.0%,在正常组织中阳性率为13.7%,表达明显增高(P<0.01);Spearman等级相关分析结果显示两者在大肠癌组织中表达呈现负相关(rs=-0.269,P<0.01)。结论 E-cadherin的表达降低和MMP-2的表达增高与大肠癌的生物学行为密切相关,两者表达水平的高低可作为临床判断大肠癌预后有价值的指标。Objective To investigate the expression and clinical significance of E-cadherin and matrix metalloproteinase-2 in cases with colorectal carcinoma. Methods Expression of E-cadherin and matrix metalloproteinase-2 in 14 cases of adenomas,102 cases of colorectal carcinomas and adjacent normal tissues was detected by immunohistochemical staining.The relationship of expression of E-cadherin and MMP-2 with the occurrence and development of CRC and the relativity in CRC were analysed. Results The positive rate of E-cadherin in normal colorectal mucous was 68.6%, the positive rate in CRC was 42.2%,the expression was significantly decreased（P〈0.01 ）; in contrast the positive rate of MMP-2 in CRC was 50.0%, the positive rate in normal colorectal mucous was 13.7%,the expression was significantly increased （P〈0.01）; Spearman analysis showed that the inverse relationship of E-cadherin and MMP-2 in CRC was observed （r =-0.269, P〈0.01）. Conclusions The downregulation of E-cadherin and the upregulation of MMP-2 are correlated with the biological behavior of colorectal carcinoma and they are valuable makers for predicting clinical prognosis of colorectal carcinoma.
