摘 要:目的探讨原发性肝细胞癌白细胞介素(IL)-17及IL-17受体(IL-17R)的表达及与临床病理的关系。方法选择2011年1月至2014年8月40例原发性肝细胞癌患者为研究对象(肝细胞癌组),并选择同期肝纤维瘤患者40例(肝纤维瘤组)和体检健康者40例(对照组)作为对照。采用免疫组化方法检测不同组织中IL-17、IL-17R的表达,并对IL-17、IL-17R的表达与原发性肝细胞癌临床病理的关系进行统计学分析。结果肝细胞癌组织IL-17、IL-17R的表达高于癌旁组织和肝纤维瘤,差异均有统计学意义(P均<0.01);肝细胞癌IL-17、IL-17R的表达阳性率(75.0%、70.0%)高于癌旁组织(47.5%、32.5%)和肝纤维瘤(37.5%、35.0%),差异均有统计学意义(P均<0.01);肝细胞癌IL-17、IL-17R的表达与原发灶分期、分化程度及淋巴结转移有关(P均<0.05)。肝细胞癌患者术前血清IL-17水平高于对照组,差异有统计学意义(P<0.01),术后IL-17水平低于术前,差异有统计学意义(P<0.01)。结论 IL-17、IL-17R在原发性肝细胞癌中呈高表达;IL-17、IL-17R可能参与了肝细胞癌的发病、转移和进展。Objective To study the expressions of Interleukin ( IL)-17 and IL-17 receptor (IL-17R) in primary hepatocellular carcinoma and their correlations with clinicopathological features of tumor. Methods Forty patients with primary hepatocellular carcinoma admitted in our hospital between January 2011 and August 2014 were selected as the research objects (hepatocellular carcinoma group) ,and 40 cases of liver fibroma (liver fibroma group) and 40 healthy subjects (control group) at the same time were selected as control. The expressions of IL-17 and IL-17R in liver tissues were measured by immunohistochemical method and their correlations with clinicopathological features of primary hepatocellular carcinoma were statistically analyzed. Results The expressions of IL-17 and IL-17R in primary hepatocellular carcinoma were signifi- cantly higher than those in the tissues adjacent to carcinoma and liver fibroma( all P 〈 0.01 ), and their positive expression rates in primary hepatocellular carcinoma (75.0% ,70.0% ) were significantly higher than those in the tissues adjacent to carcinoma (47.5% ,32.5% ) and liver fibroma (37.5% ,35.0% ). The expressions of IL-17 and IL-17R in primary hepatocellular carcinoma were correlated with the staging and differentiation degree of primary foci and lymph node metastasis (all P 〈0.05 ). Preoperative serum IL-17 level in primary hepatocellular carcinoma patients was significantly higher than that in control group( P 〈 0.01 ), and the postoperative serum IL-17 level dropped obviously compared with preoperative level (P 〈0.01 ). Conclusion IL-17 and IL-17R present high-expression in primary hepatocellular carcinoma, and they might be involved in the occurrence, metastasis and development of primary hepatocellular carcinoma.
