摘 要:目的:探讨咖啡因在超低出生体重儿机械通气撤机过程中的作用,以缩短撤机时间,提高安全性,降低并发症。方法:选取2013年5月至2014年11月70例超低出生体重儿患者为研究对象,均予机械通气,在撤机观察中对照组予氨茶碱,观察组予咖啡因,观察在相关指标上变化情况。结果:疗效上,对照组显效率40%,总有效率85.71%;观察组显效率80%,总有效率100%,两组比较差异有统计学意义(P<0.05);并发症上,对照组并发症总发生率为20%,观察组并发症发生率为5.71%,两组比较差异有统计学意义(P<0.05);两组在心率、收缩压、中心静脉压、肺动脉压上比较差异无统计学意义(P>0.05);而在心排血量指数、尿量、呼吸频率、氧合指数、动脉血二氧化碳分压、p H值比较差异有统计学意义(P<0.05)。结论:咖啡因能降低超低出生体重儿机械通气撤机后并发症,安全性高。Objective:To investigate the effects of caffeine in low birth weight children during weaning from mechanical venti-lation, in order to shorten the weaning time, improve safety and reduce complications. Methods:From May 2013 to November 2014 70 cases of low birth weight children were selected for the study, therefore correspondingly mechanical ventilation weaning observation in the control group were treated with theophylline, caffeine observation group were observed in the relevant changes in the indicators. Re-sults:The efficacy and efficiency of the control group was 40%, the total efficiency of 85. 71%;observation group was 80% efficiency, the total efficiency of 100%, the difference was statistically significant (P〈0. 05);the complications, control group total complication rate was 20% in the observation group complication rate was 5. 71%, the difference was statistically significant (P〈0. 05);compare the two groups in heart rate, systolic blood pressure, the central venous pressure, pulmonary artery pressure the difference was not sta-tistically significant (P〉0. 05);cardiac output and cardiac index, urine, respiratory rate, oxygenation index, arterial carbon dioxide partial pressure, PH value of the difference was statistically significant (P〈0. 05). Conclusion:Caffeine can reduce complications of low birth weight children after weaning from mechanical ventilation.
