伽玛刀及射波刀放射外科治疗三叉神经痛的疗效对比分析

摘　　要：目的研究伽玛刀及射波刀放射外科治疗三叉神经痛的有效性和安全性。方法对随访时间超过12个月同期分别行伽玛刀或射波刀放射外科治疗的三叉神经痛患者进行回顾性分析,总结患者的疼痛控制起效时间、疼痛控制率及并发症发生率。结果共计48例接受伽玛刀治疗和19例接受射波刀治疗的患者纳入此研究,伽玛刀组疼痛控制起效中位时间为治疗后36天(1~180天),射波刀组为治疗后41天(1~158天),两者无统计学差异(P=0.281)。伽玛刀组疼痛控制有效者(I^III级)共计41例(85.4%);射波刀组疼痛控制有效者(I^III级)共计15例(78.9%),两组之间无统计学差异(P=0.492)。面部感觉异常发生率在伽玛刀组有7例(14.6%),射波刀组有4例(21.1%)。结论伽玛刀与射波刀放射外科治疗三叉神经痛效果相当,疗效确切,不良反应发生率低,是不能耐受手术患者的较好的治疗方式。Objective To study the efficacy and safety of gamma knife and CyberKnife radiosurgery for trigeminal neuralgia（TN）.Methods Retrospectively analyzed the trigeminal neuralgia patients who received gamma knife or CyberKnife radiosurgery treatment at the same time period and assessed the safety and efficacy.Results There are 48 patients underwent gamma knife therapy and 19 cases treated with CyberKnife.The median pain relief time was 36 days or 41 days for gamma knife or CyberKnife treatment.No significant differences were found in the two groups（P=0.281）.The pain control rate for gamma knife group and CyberKnife group were 85.4% and 78.9%,respectively.There are no significant differences in the two groups（P=0.492）.Facial sensory dysfunction occurred in 14.6% patients of gamma knife group and it was 21.1%in CyberKnife group,no serious treatment-related complications were found.Conclusion Gamma knife and CyberKnife radiosurgery have the same efficiency for trigeminal neuralgia and they should be considered as an alternative option for the patients who cannot tolerate invasive surgery.
