摘　　要：目的探讨血管内皮生长因子(vascular endothelial growth factor,VEGF)对恶性肿瘤的诊断价值。方法 448例不同恶性肿瘤患者为恶性肿瘤组,112例体检健康者为对照组,2组均采用化学发光法检测血清VEGF水平,比较VEGF在不同种类肿瘤中的表达情况及阳性率,分析VEGF对肿瘤检测的灵敏度及特异性,及其与组织病理学检查结果的一致性。结果恶性肿瘤组血清VEGF水平[(286.6±87.5)ng/L]明显高于对照组[(93.6±28.5)ng/L],差异有统计学意义(P<0.05);VEGF诊断胰腺癌、胃癌、肾癌、结直肠癌及子宫内膜癌阳性率均>70%;VEGF对肿瘤检测的灵敏度为70.09%、特异性为89.29%,与组织病理学检查结果有较高一致性(Kappa值=0.422)。结论 VEGF在肿瘤早期筛查方面有一定临床诊断价值,有可能成为肿瘤筛查的广谱肿瘤标志物之一。Objective To explore the clinical value of vascular endothelial growth factor （VEGF） to the diagnosis of malignant tumors. Methods The VEGF level was detected by chemiluminescent immurioassay in 448 various malignancies （malignant tumor group） and 112 healthy volunteers （control group）. The expressions of VEGF in various malignancies and their positive rates were compared, and the sensitivity and specificity of VEGF detection method and its consistency with pathological diagnosis results were analyzed. Results The serum VEGF level of malignant tumor group （（286.6±87.5） ng/L） was significantly higher than that in control group （（93.6±28.5） ng/L） （P〈0.05）. The VEGF positive rates of pancreatic cancer, stomach cancer, kidney cancer, colorectal cancer and endometrial cancer were over 70%. The sensitivity and specificity of VEGF for cancer were 79. 09% and 89. 29% respectively, and had a high consistency with pathological results （Kappa=0. 422）. Conclusion VEGF has a certain clinical diagnostic value to the early detection of tumor, and is expected to become a kind of broad-spectrum tumor markers for cancer screening.
