摘 要:目的探究7~10岁完全性腭裂儿童术后/a/、/i/、/u/的嗓音声学特点,为腭裂儿童言语康复治疗提供参考。方法选取7~10岁普通儿童17名和完全性腭裂术后儿童18名,比较两组儿童/a/、/i/、/u/的基频F。、jitter、shimmer、标准化噪声能量(normalized noise energy,NNE)、谐噪比(harmonic-noiseratio,HNR)等电声门图参数的差异。结果①两组儿童/a/的F。均显著小于/u/,/a/的jitter均显著大于/u/,/a/的shimmer分别显著、极显著大于/i/、/u/;②普通儿童/a/、/i/、/u/间的NNE、HNR值不存在显著性差异,完全性腭裂儿童/a/的NNE极显著大于/i/、/u/,其/a/的HNR极显著大于/u/。③完全性腭裂儿童/a/、/i/、/u/的基频F。均显著小于普通儿童(尸〈0.05),/a/、/i/、/u/的jitter均显著大于普通儿童(尸〈0.05);/a/的NNE显著大于普通儿童、/a/的HNR显著小于普通儿童(P〈0.05)。结论①两类儿童发/a/时声带振动规律性相对/i/、/u/更差;②术后完全性腭裂儿童发/a/时声门闭合程度相对/i/、/u/更小;③术后完全性腭裂儿童发声时声带振动速度相对普通儿童更慢,声带振动规律性更差,发/a/时声门闭合程度相对更小;④临床上可将敏感性较高的/a/音作为腭裂儿童嗓音音质的测试语料;在制订嗓音康复策略时应综合考虑腭裂儿童不同舌位对声带振动的影响。Objective To explore the postoperative voice characteristics of/a/,/i/and/u/in 7-10-year-old children with complete cleft palates and to provide reference for the speech rehabilitation of these children. Methods Eighteen children of 7-I0 years old with complete cleft palates after operation and seventeen normal controls of similar ages were recruited in this study. The electroglottographic parameters of/a/,/i/ and /u/ were compared between the two groups of children. Results (1) For both groups, F0 of/a/ was significantly lower than that of/u/, jitter of / a/ was significantly higher than that of/u/ and shimmer of/a/was significantly higher that of/i/and/u/.(2)There was no significant difference in normalized noise energy (NNE) and harmonic-noise ratio (HNR) among /a/, /i/ and/u/ of normal children. For children with complete cleft palates after operation, NNE of/a/ was significantly higher than that of/i/ and /u/, and HNR of/a/ was significantly higher than that of/u/. (3) The F0S of/a/,/i/ and/u/ in children with complete cleft palates after operation were significantly lower than those of normal children (P〈0.05), jitters of/a/,/i/and/u/and NNE of/a/were significantly higher than those of normal children (P〈0.05) and HNR of/a/was significantly lower than that of normal children(P〈0.05). Conclusion (1) The normal vocal fold vibration are influenced by tongue position for the two groups of children. The vibration is more irregular when pronouncing/a/than/i/and/u/. (2)The degree of glottal closure is smaller for the children with complete cleft palates after operation when pronouncing /a/ than /i/ and /u/. (3) Generally, the vocal fold vibration of children with complete cleft palates after operation is slower and more irregular, and the degree of glottal closure is smaller than that of normal children when pronouncing/a/. (4) In clinical practice,/a/can be used as the assessment material when assessing the voice quality of children with cleft palates. The eff
