摘  要:目的了解社区女性宫颈高危型人乳头瘤病毒(HR-HPV)感染状况,HR-HPV感染亚型与宫颈病变严重程度的相关性,为预防人乳头瘤病毒(HPV)感染和宫颈癌防治提供可靠的科学依据。方法将6258例参加社区健康普查妇女的宫颈脱落细胞进行HPV基因分型检测及液基细胞学检测(LCT),两项检测阳性者行阴道镜下活检,病理结果为诊断标准。结果 6258例标本中HR-HPV阳性者643例,感染率为10.27%,其中单一型感染534例,感染率8.53%;多重感染109例,感染率1.74%;各年龄段HR-HPV感染率分别为20~30岁(9.72%)、31~40岁(7.68%)、41~50岁(8.67%)、〉50岁(11.29%),HR-HPV阳性中,HPV 52型感染率最高2.73%,其次为HPV 16、58、68、51。LCT结果≥不典型鳞状上皮细胞(ASCUS)的女性77例行阴道镜检查,在阴道镜下对可疑处行多点活检并进行病理诊断,发生高度病变CINⅡ、CINⅢ共18例,其中HPV 16型感染占7例、52型5例、58型4例、18型2例。结论 HPV基因分型检测对持续感染人群的追踪随访更有价值,建议对社区妇女应加强宣教,促进健康人群定期筛查能有效预防宫颈癌前病变的发生。Objective To understand infection condition of uterine cervix high risk human papillomavirus(HR-HPV) in community women, and correlation between subtype of HR-HPV infection and severity of cervical lesions, in order to provide reliably scientific reference for prevention and treatment of human papillomavirus(HPV) infection and cervical cancer. Methods Cervical exfoliated cells in 6258 women in community health screening were taken in HPV genotyping detection and liquid-based cytology test(LCT). Positive patients in these two tests received colposcopic biopsy, and their pathological results were taken as diagnosis criteria. Results There were 643 HR-HPV positive cases among 6258 cases, with the infection rate as 10.27%. Among them, there were 534 simple infection cases with infection rate as 8.53% and 109 multiple infection with infection rate as 1.74%. HR-HPV infection rates in age groups were 9.72% in 20~30 years old, 7.68% in 31~40 years old, 8.67% in 41~50 years old, and 11.29% in 50 years old. In positive HR-HPV cases, HPV52 type had the highest infection rate as 2.73%, followed by HPV16, 58, 68, 51. There were 77 cases with LCT ≥ atypical squamous cells of undetermined significance(ASCUS) receiving colposcopy, and pathological diagnosis was made by colposcopic multi-point biopsy on suspected area. A total of 18 cases had high lesion CIN Ⅱ and CIN Ⅲ. Among them there were 7 cases with HPV16 type infection, 5 cases with 52 type infection, 4 cases with 58 type infection, and 2 cases with 18 type infection. Conclusion HPV genotyping detection contains more value in follow-up for persistent infection population. Strengthened education and promoted regular screening in health people are recommended for effective prevention of precancerous lesions of uterine cervix.
