摘 要:目的:探讨白内障术后屈光状态特点及术前前房深度(anterior chamber depth,ACD)与术后屈光状态的关系分析。方法:将96例96眼白内障患者随机分为两组,超声乳化组患者采用超声乳化吸除联合人工晶状体(intraocular lens,IOL)植入术治疗,小切口组患者采用小切口囊外摘除联合IOL植入术治疗。统计两组患者手术前后前房深度变化情况及术后屈光状态和屈光完全矫正值,分析其相关性。结果:超声乳化组患者术后ACD增厚0.74mm,小切口组患者术后ACD增厚0.78mm,术后两组患者ACD比较差异无统计学意义(P>0.05)。超声乳化吸除联合IOL植入术和小切口囊外摘除联合IOL植入术后,两组患者ACD均明显加深(P<0.05),且两组患者术后矫正视力均明显优于裸眼视力(P<0.05)。两组患者术后屈光度主要为0^+1.0D(41.67%和54.16%)和+1.25^+2.0D(43.75%和33.33%)(P>0.05)。结论:白内障IOL植入术后患者ACD明显加深,临床需充分考虑ACD的变化情况,提高IOL屈光度数计算准确性。AIM : To study the refractive status characteristics aftser cataract surgery and the correlation between preoperative anterior chamber depth( ACD) and refractive status.METHODS: Ninety-six cases of patients with cataract were randomly divided into two groups. The patients in phacoem ulsification group were treated with phacoemulsification com bined intraocular lens( IOL)im plantation while the patients in sm all incision group were treated by small incision extracapsular cataract extraction com bined with IOL im plantation. Changes in ACD and postoperative refractive status and refractive fully corrected value were counted and the correlation of them were analyzed. RESULTS: ACD of the phacoemulsification group deepened 0. 74 mm while that of the sm all incision group deepened 0. 78 m m after treatm ent and there was no significant difference( P〉0. 05). After operation,the ACD of two groups significantly deepened( P 0. 05). The postoperative visual acuity of two groups were significantly better than the uncorrected visual acuity of tw o groups( P〈0. 05). The postoperative refraction of two groups patients were mainly 0 - + 1. 0D( 41. 67% and54. 16%) and +1. 25 - +2. 0D( 43. 75% and 33. 33%)( P〉0. 05). CONCLUSION: ACD is significant deepened afteroperation. Surgeon needs full consideration of changes to improve the refractive IOL calculation accuracy.
