乳腺癌及乳腺良性肿块的MRI征象与E-cadherin蛋白表达相关性研究

摘　　要：目的探讨乳腺癌及乳腺良性肿块的MRI征象与E-cadherin表达的相关性。方法搜集2012年至2014年术后确诊为乳腺癌20例及21例良性肿块患者资料。患者术前均行MRI检查(平扫、DWI及增强),并对MRI特征,如病灶的形态、ADC值、早期强化、强化曲线类型及淋巴结转移进行分析。术后标本先做常规病理检查,然后采用免疫组织化学(S-P)法,用抗E-cadherin抗体对标本进行标记分析,结果乳腺良性肿块的MRI征象(圆形、规则、ADC值高、Ⅰ型强化)与E-cadherin的正常表达相关,有显著性统计学意义。乳腺癌的MRI征象(分叶、不规则、ADC值降低、Ⅱ型强化及Ⅲ型强化、淋巴结转移)与E-cadherin的正常表达降低相关,有显著性统计学意义。结论乳腺癌及良性肿块的MRI征象与E-cadherin的正常表达密切相关,可以间接地反映乳腺癌及良性肿块的某些分子生物特性。Objective To explore the relationship between MRI features and expression of E cadherin in breast carcinoma and benign tumors. Methods Cases postoperatively diagnosed with breast cancer in 20 cases and benign tumor in 21 cases were collected from 2012 to 2014,These patients were examined by MRI（ plain scan,DWI and enhanced）,MRI features,such as shape,ADC value,early intensive,enhancement curve type of the lesion and the lymph node metastasis were Analyzed. Postoperative samples underwent routine pathological examination,the immunohistochemical（ S-P） method was used to test the expression of E-cadherin in the samples,then analyzed the relationship of MRI features and expression of Ecadherin of 20 cases of breast cancer and 21 cases of benign tumor samples. Results MRI features of benign breast masses（ round,rules,high ADC value,type I strengthening） associated with normal E-cadherin expression was statistically significant. MRI signs of breast cancer（ leaf,irregular,low ADC value,type II and III strengthening、lymph node metastasis）associated with the decreased expression of E-cadherin,was statistically significant. Conclusion MRI signs of breast cancer and benign masses is closely correlated with the expression of E-cadherin,may indirectly reflect certain molecular biological characteristics of breast cancer and benign tumor.
