摘 要:目的该研究拟在内蒙古通辽地区的蒙古族人群中验证二甲基精氨酸-二甲胺水解酶2(DDAH2)基因多态性与高血压左心室肥厚的关联。方法研究对象为2011年9月-2013年5月通辽市疾病预防控制中心收集的800例蒙古族高血压患者,其中包括400例高血压伴左-42室肥厚(LVH)患者,400例高血压不伴有LVH患者,同时收集400例血压正常人作为对照。所有入选者均进行心脏超声的检测。使用单倍体型-tagging-单核苷酸的多态性(SNPs)的方法,研究DDAH2基因tagSNPs与原发性高血压继发左心室肥厚遗传易感因素的关系。结果在DDAH2基因上与其他SNPs(rs707916,rs805304)完全连锁,位于启动子区的tag-SNP-449C/G(rs805305),与携带GG基因型的患者相比,携带C等位基因(GC+CC)的高血压左心室肥厚患者的左心室质量指数[(61.5±11.2)g/m2.7 vs(56.9±11.1)g/m2.7,P〈O.01]、左心室后壁厚度[(12.1±2.2)mmvs(11.0±2.0)mm,P〈0.051以及相对室壁厚度f(48.1±9.0)%VS(45.828.7)%,P〈0.011均增加。此外,-449位携带C等位基因(GC+CC)增加高血压左心室肥厚的易感度(OR=1.33,95%CI:1.01-1.45,P〈O.05)。结论DDAH2基因的常见变异-449C/G与蒙古族高血压继发左心室肥厚的发生有关联。[ Objective] To explore the relationship between the common variant of dimethylarginine dimethy- laminohydrolase 2 (DDAH2) gene and left ventricular hypertrophy (LVH) of patients with essential hypertension in Tongliao of Inner Mongolia. [Methods] The case-control study consisted of 400 hypertensive patients with LVH and 400 hypertensive patients without LVH, as well as 400 control subjects. A haplotype-tagging single nucleotide polymorphisms (SNPs) approach was used to identify tag SNPs in DDAH2. The tag SNPs were genotyped in case- control study. [ Results ] A promoter variant -449C/G (rs805305) was identified and found to be the only tag-SNP in completely linkage disequilibrium with other SNPs (rs707916, rs805304) in the region containing DDAH2. Compared with the carriers of the GG genotype, the hypertensive patients with LVH carrying the C allele (GC+CC) had an increase in the LV mass index [(61.5±11.2) g/m2.7 vs (56.9±11.1) g/m2.7, P 〈 0.01], posterior wall thickness [(12.1±2.2) mm vs (11.0±2.0) mm, P 〈 0.05], and relative wall thickness [(48.1±9.0)% vs (45.8±8.7)%, P 〈 0.01]. In addition, the C allele (GC+CC) of the -449 locus was associated with higher risk of LVH (OR=1.33, 95%CI: 1.01-1.45, P 〈 0.05) in the study. [ Conclusions ] DDAH2 common variant is associated with LVH in essential hypertensive patients in Tongliao of Inner Mongolia.
