摘 要:目的探究小于胎龄儿(SGA)宫外生长发育迟缓(EUGR)与血浆游离肉碱(FC)水平的相关性。方法选择2013年1~12月在佛山市妇幼保健院出生的SGA113例为实验组,根据出生体重(BW)划分为A组(Bw≤1500g)及B组(BW〉1500g),测量并根据其生后第28天的体重,分为EUGRA组51例,非EUGRA组15例,EUGRB组33例,非EUGRB组14例。另选取同时期在该院出生的正常足月儿(37周≤胎龄〈42周)20例作为对照组。应用串联质谱分析仪检测所有对象生后6h内及第7天、14天、21天、28天血浆FC水平。结果以体重为标准,实验组EUGR发生率为74.3%(84/113),各组FC随时间变化呈先下降后上升的趋势。对照组Fc在第7天降为最低,下降幅度为24%,A组在第14天降为最低,下降幅度为44%(EUGR)及45%(非EUGR),B组在第14天已呈上升趋势,下降幅度为38%(EUGR)及32%(非EUGR)。对照组在28天时超过出生时水平,实验各组在28天时均未恢复到出生时水平。EUGR组FC值均明显低于同体重级别非EUGR组(P〈0.05)。结论SGA宫外生长发育迟缓发生率较高,其血浆FC值明显低于同组宫外生长发育正常儿,且FC下降持续的时间及幅度可能在这一过程中起重要作用。Objective To explore the correlation between extrauterine growth retardation (EUGR) and free earnitine (FC) of small for gestational age (SGA) infants. Methods A total of 113 SGA infants bern in the hospital from January to December in 2013 were selected as experimental group, then the infants were divided into different subgroups according to birth weight: group A (equal to or less than 1 500 g) and group B ( more than 1 500 g) ; then the infants in group A and group B were further divided into different subgroups according to the weight on the 28th day after birth: EUGR A group (51 infants), non-EUGR A group ( 15 infants), EUGR B group (33 in- fants), non-EUGR B group ( 14 infants) . Twenty normal term infants (gestational age was equal to or more than 37 gestational weeks, less than 42 gestational weeks) bern in the hospital during the same period were selected as control group. Tandem mass spectrometry was used to detect serum FC levels within 6 hours and at 7, 14, 21, and 28 days after birth. Results Taking weight as standard, the incidence rate of EUGB in experimental group was 74. 3% (84/113 ), FC levels in different groups decreased and then increased with time. In control group, FC level on the seventh day was the lowest, the decreasing amplitude was 24% ; in group A, FC leveIs on the fourteenth day were the lowest, the decreasing amplitudes were 44% in EUGR group and 45% in non-EUGR group, respectively; in group B, FC levels on the fourteenth day had already increased, the decreasing amplitudes were 38% in EUGR group and 32% in non-EUGR group; in control group, FC level on the 28th day was higher than that at birth; in experimental group, FC levels on the 28th day had not returned to the levels at birth. FC levels in EUGR group were statistically significantly higher than those in non-EUGR group (P〈0. 05) . Conclusion The incidences rate of EUGR in SGA infants is relatively high.
