前牙区即刻种植即刻自体冠修复种植体的临床效果

摘　　要：目的对比观察前牙区即刻与延期种植即刻自体冠修复种植体的临床效果。方法选择23例需要行前牙区种植修复的病人,其中A组11例前牙外伤的病人行不翻瓣即刻种植即刻自体冠修复11枚Noble种植体,B组12例前牙缺失的病人行牙种植并延期修复12枚Noble种植体,均于术后6个月行永久冠修复。分别对A、B组的牙龈乳头指数（PIS）、红色美学指数（PES）和种植体周围骨组织量进行测量、分析。结果 23枚种植体均无脱落,A组种植前、永久冠修复后即刻和永久冠修复12个月后的PIS和PES无明显差异（P〉0.05）;B组永久冠修复后即刻较永久冠修复12个月后PIS和PES无明显差异（P〉0.05）;A组永久冠修复后即刻和修复后12个月的PIS和PES与B组比较差异有显著性（χ2=7.22~14.31,P〈0.05）;永久冠修复后即刻和修复后12个月种植体周围的骨吸收量A、B组比较差异无显著性（P〉0.05）。结论即刻种植即刻自体冠修复种植体对前牙具有一定的临床美学效果。Objective To compare the effectiveness between immediate implantation plus immediate autologous tooth crown restoration and delayed implantation on bone and soft tissue around anterior teeth area. Methods Twenty-three patients who needed undergo implantation and prothesis in anterior teeth area were enrolled.In group A,11 patients with anterior teeth injury underwent 11 immediate Noble implants and immediate autologous teeth crown restoration;in group B,12 patients with loss of anterior teeth received dental implantation,and 12 delayed Noble implants.Permanent crown restorations in both groups were performed six months later.The gingival papilla index score（PIS）,pink esthetic score（PES）and bone absorption around implants in groups A and B were measured and analyzed,respectively. Results No falling off was noted in 23 implants.The PIS and PES in group A-before implantation,after immediate crown restoration,and 12 months later showed no significant difference（P0.05）;that in group B were also no significant difference（P0.05）.The PIS and PES between groups A and B showed significant difference in terms of immediate crown restoration and restoration 12 months later（χ2=7.22-14.31,P0.05）,and bone absorption between the two groups was no significant difference（P0.05）. Conclusion Immediate implant and immediate autologous tooth crown restoration have a certain esthetic effect on anterior teeth.
