早期胃癌hMLH1和hMSH2基因表达及意义

摘　　要：目的探讨早期胃癌胃黏膜组织hMLH1和hMSH2蛋白的表达及其意义。方法选取内镜下切除早期胃癌组织45例、癌前病变组织38例以及胃镜下活检慢性胃炎组织42例,采用免疫印迹法检测hMLH1和hMSH2蛋白的表达。结果慢性胃炎及癌前病变组织hMLH1蛋白表达高于早期胃癌组织,差异有统计学意义(F=9.578,P<0.05);早期胃癌组织hMSH2蛋白表达高于癌前病变与慢性胃炎组织,差异有统计学意义(F=32.952,P<0.05)。早期胃癌病人hMSH2及hMLH1蛋白表达与年龄、性别、肿瘤部位、肿瘤大小、浸润深度、组织学类型等无关(P>0.05)。幽门螺杆菌(Hp)感染组早期胃癌组织hMSH2阳性表达率低于非Hp感染组(χ2=22.361,P<0.05),癌前病变、慢性胃炎组织hMSH2阳性表达率也低于非Hp感染组,但差异无统计学意义(P>0.05);Hp感染组早期胃癌、癌前病变和慢性胃炎组织hMLH1阳性表达率均低于非Hp感染组,但差异无统计学意义(P>0.05)。结论在慢性胃炎、癌前病变和早期胃癌的发展过程中,hMSH2基因表达增加,hMLH1基因表达减少。Hp感染可能通过影响hMSH2与hMLH1基因表达,影响早期胃癌的发生。Objective To explore the expressions of hMLH1 and hMSH2proteins in mucous membrane of early-stage gastric cancer and their significance. Methods Samples from 45 cases of early gastric cancer resected through endoscopic surgery,and 38 cases of premalignant lesions were collected,and 42 cases of chronic gastritis confirmed by gastroscopic biopsies were collected as well.The expressions of hMLH1 and hMSH2in three kinds of the tissues were detected using Western blotting. Results The expression of hMLH1 in chronic gastritis and precancerous lesions was higher than that in early gastric cancer,the difference was statistically significant（F=9.578,P〈0.05）;the expression of hMSH2 in early gastric cancer was higher than that in chronic gastritis and precancerous lesions（F=32.952,P〈0.05）.The expressions of hMLH1 and hMSH2protein in early gastric cancer were not correlated with patient＇s age and sex,location and size of tumor,depth of infiltration and histological type（P〉0.05）.The positive rate of hMSH2 in early gastric cancer infected with Hp was lower than that in non-Hp-infected（χ^2=22.361,P〈0.05）,the expression of hMSH2 in precancerous lesions and chronic gastritis was also lower than that in non-Hp-infected group,but the difference was not statistically significant（P〉0.05）. Conclusion In the development process from chronic gastritis to precancerous lesions to early gastric cancer,the expression of hMSH2 increases and that of hMLH1 decreases.Hp infection impacts the occurrence of early gastric cancer probably through affecting the expressions of both hMSH2 and hMLH1.
