摘　　要：目的探讨CT引导125I放射性粒子植入治疗放疗后复发颈部淋巴结转移瘤近期效果。方法采用CT引导下经皮穿刺组织间植入125I放射性粒子方法,治疗放疗后复发颈部淋巴结转移瘤的病人48例,淋巴结总数69个,直径1.5~5.5cm。结果术后2个月复查,淋巴结完全缓解(CR)55个,部分缓解(PR)9个,无变化(NC)5个,进展(PD)0个,有效率为92.75%。术后6个月复查,CR者57个,PR者8个,NC者3个,PD者1个,有效率为94.20%。无病人死亡,无病人出现血管栓塞。术后1个月内局部皮肤红肿8例,未经特殊处理自愈;术后6个月局部色素沉着10例。结论 CT引导经皮穿刺125I放射粒子植入治疗放疗后复发颈部淋巴结转移瘤近期效果好,创伤小,并发症发生率低。Objective To evaluate the short term efficacy of CT guided 125 I seed implantation for lymph node metastatic tumors of the neck after radiotherapy. Methods Employing percutaneous CT-guided interstitial implantation of 125 I to treat 48 patients with recurrent cervical lymph node metastatic tumors after radiotherapy,a total of 69 lymph nodes（diameter ranging1.5-5.5mm）was included. Results A recheck of two months after surgery showed complete remission（CR）in 55 lymph nodes,partial remission（PR）in nine,no change（NC）in five,and no lymph nodes with progression,effective rate being 92.75%.A review after 6months showed CR in 57 lymph nodes,PR in eight,NC in three and progression in one,the effectiveness being94.20%.No death and vascular embolism were noted during 6-month follow-up.Local skin red and swollen was found in eight patients within one month after surgery,all healed untreated.Ten patients with local pigmentation of the skin were seen 6months after therapy. Conclusion Percutaneous CT-guided implantation of 125 I can get a good short-term outcome with little trauma and low complications in the treatment of recurrent lymph-node metastatic tumors of the neck after radiotherapy.
