摘  要:目的探讨高迁移率族蛋白B-1(HMGB-1)在急性胰腺炎大鼠模型中的表达及其意义。方法将200只Sprague-Dawley(SD)大鼠随机分入假手术组、0.9%氯化钠溶液组、轻症急性胰腺炎(MAP)组和重症急性胰腺炎(SAP)组,每组50只。通过各组大鼠的胰腺组织病理学评分验证模型,采用ELISA法检测模型建立后6、12、18和24h各组大鼠胰腺组织和血清中HMGB-1的水平。结果 SAP组和MAP组与假手术组间大鼠改良Schmidt评分构成比的差异均有统计学意义(P值均<0.05),MAP组与SAP组间的差异亦有统计学意义(P<0.05)。假手术组造模后18h时大鼠胰腺组织和血清中HMGB-1水平均显著低于MAP组和SAP组同时间点(P值均<0.05)。SAP组造模后18h时大鼠胰腺组织和血清中HMGB-1水平均显著高于同组造模后6h时(P值均<0.05),低于同组造模后24h时(P值均<0.05)。MAP组和SAP组造模后6h时的血清淀粉酶水平均显著低于同组造模后18h和24h时(P值均<0.05),MAP组和SAP组造模后18h时的血清淀粉酶水平均显著低于同组造模后24h时(P值均<0.05);MAP组和SAP组造模后12、18、24h时的血清淀粉酶水平均显著高于假手术组同时间点(P值均<0.05)。结论大鼠胰腺组织和血清中HMGB-1表达水平与胰腺炎的严重程度一致,有可能成为评价胰腺炎病情严重程度和判断预后的临床指标。Objective To investigate the expression of high mobility group box-1 (HMGB-1) in severe acute pancreatitis rats. Methods Two hundred Sprague-Dawely (SD) rats were randomly divided into 4 groups (n =50); sham operation group, normal saline group, mild acute pancreatitis (MAP) group and severe acute pancreatitis (SAP) group. Pancreatic tissues of rats in each group were evaluated histologically. HMGB-1 expression in pancreatic tissue and serum were measured by enzyme-linked immunosorbent assay (ELISA) at 6 h, 12 h, 18 h and 24 h after modeling. Results There were significant differences in the pathological score of pancreatic tissue (Schmidt score) between sham operation group, SAP group and MAP group (all P〈0.05). The levels of HMGB-1 in both pancreatic tissue and serum in SAP and MAP group were significantly higher than that in sham operation group 18 h after modeling (both P〈0.05). Compared with that at 18 h after modeling, the levels of HMGB-1 in both pancreatic tissue and serum were significantly decreased at 6 h after modeling while increased at 24 h after modeling in SAP group (all P^0.05). Serum amylase in SAP group and MAP group at 6 h after modeling were significantly lower than those at 18 h and 24 h after modeling (all P〈0.05), and the serum amylase in SAP group and MAP group at 18 h after modeling were also significantly lower than those at 24 h after modeling (both P〈0.05). Serum amylase in SAP group and MAP group at 12 h, 18 h and 24 h after modeling were significantly increased as compared with those in sham operation group (all P〈0.05). Conclusion HMGB-1 expression in both pancreatic tissue and serum are positively correlated with the severity of pancreatitis in rats, so it may serve as an index to evaluate disease condition and prognosis of acute pancreatitis.
