外周血CD4^+CD25^+CD127^(low)调节性T细胞在恶性肿瘤患者免疫功能评估中的作用

摘　　要：目的观察恶性肿瘤患者外周血CD4+CD25+CD127low调节性T细胞(CD4+CD25+CD127lowTreg)的数量变化,探讨外周血CD4+CD25+CD127lowTreg检测在恶性肿瘤患者免疫功能评估中的作用。方法以35例正常人作为对照组,采用流式细胞术检测74例肿瘤患者外周血CD4+CD25+CD127lowTreg在CD4+T细胞的比例,分析不同TNM分期恶性肿瘤患者外周血中CD4+CD25+CD127lowTreg占CD4+T细胞的百分比,采用实时荧光定量RT-PCR技术检测PBMC转录因子Foxp3表达水平。结果肿瘤患者外周血中CD4+CD25+CD127low Treg占CD4+T细胞百分比为(6.19±1.82)%,明显高于对照组(3.12±1.16)%,差异具有统计学意义(P<0.05)。其中Ⅲ、Ⅳ期患者外周血CD4+CD25+CD127lowTreg百分比分别为(6.08±2.14)%、(6.88±2.65)%,明显高于Ⅰ-Ⅱ期患者(5.65±1.86)%,P<0.05;病理分型为低分化者CD4+CD25+CD127lowTreg百分比为(6.72±2.60)%,明显高于高分化者(5.94±2.11)%,P<0.05;有淋巴结转移者CD4+CD25+CD127lowTreg百分比为(6.95±3.12)%,明显高于无淋巴结转移者(5.02±2.09)%,P<0.05。结论恶性肿瘤患者外周血CD4+CD25+CD127lowTreg占CD4+T细胞的比例明显升高,监测CD4+CD25+CD127lowTreg有利于评估肿瘤患者的免疫功能和辅助判断肿瘤患者的病情进展及预后。Objective This study is assigned to investigate the changes of CD4^＋CD25^＋CD127^lowTreg in peripheral blood from cancer patients and explore its clinical significance.Methods The proportion of CD4^＋CD25^＋CD127^lowTreg in CD4＋T cells in peripheral blood were measured by flow cytometry in 74 tumor patients and 35 cases of normal people as control.Foxp3 expression were determined by quantitative real？time PCR.Results The ratio of CD4＋CD25＋CD127lowTreg in CD4^＋T cell of tumor patients（6.19±1.82%）was obviously higher than that in the healthy control group（3.12±1.16%）,the difference was statistically significant（P〈0.05）.Peripheral blood CD4^＋CD25^＋CD127^low Treg level is significantly higher ratio in patients with stageⅢ（6.08±2.14%）and stage IV（6.88±2.65%）than that in stageⅠ-Ⅱ（5.65±1.86%）,P0.05.With pathological classification,CD4^＋CD25^＋CD127^lowTreg level in the poorly differentiated is higher than that in the high differentiated（6.72±2.60% vs 5.94±2.11,P0.05）.CD4^＋CD25^＋CD127^lowTreg level in the patients who got lymph node metastasis is higher than that in the patients who didn＇t get lymph node metastasis（6.95±3.12% vs 5.02±2.09%,P〈0.05）.Conclusion The proportion of CD4^＋CD25^＋CD127^27low Treg in peripheral blood of tumor patients increased significantly and was related to tumor progression.CD4^＋CD25^＋CD127^lowTreg would help evaluate the immune function of cancer patients.
