摘  要:目的探讨卵泡未破裂黄素化(LUF)周期对冻融胚胎移植(FET)临床结局的影响。方法回顾性分析采用自然周期准备子宫内膜的647个FET周期的临床结局,其中LUF周期69个(LUF组),同期正常排卵周期578个(对照组),比较两组患者的年龄、移植日子宫内膜厚度、移植决定日孕酮水平及上升值、胚胎种植率、临床妊娠率及流产率等。结果两组患者年龄、不育年限、移植日子宫内膜厚度、优质胚胎数等比较,差异均无统计学意义(P>0.05)。LUF组移植决定日孕酮水平为(13.99±4.26)nmol/L显著低于对照组的(15.15±4.20)nmol/L(P<0.05);LUF组移植决定日孕酮水平上升值为(6.07±2.71)nmol/L显著低于对照组的(8.57±2.71)nmol/L(P<0.05);LUF组胚胎种植率和临床妊娠率分别为11.56%、18.84%,显著低于对照组的22.85%和35.99%(P<0.05)。结论 LUF组患者孕酮水平相对较低,上升较慢,胚胎种植率及临床妊娠率均低于对照组,提示LUF可能影响胚胎着床及胚胎发育,建议临床上对发生LUF的周期取消FET。Objective. To analyze the influence of luteinized unruptured follicle (LUF)on clinical outcomes of frozen-thawed embryo transfer(FET). Methods: The data of 647 FET cycles in the natural cycle including 69 LUF cycles and 578 normal ovulation cycles were analyzed retrospectively. The patients age, endometrial thickness and serum progesterone levels on the day of dating embrgo transfer,embryo implantation rate,clinical pregnancy rate and abortion rate were compared between the two groups. Results. There were no significant differences in the patients' age, duration of infertility, the endometrial thickness and the number of good quality embryos(P〈0.05). The progesterone levels on the day of dating embryo transfer in LUF group [(13.99±4.26)vs. (15.15±4. 20)nmol/L] were significantly lower than those in normal ovulation cycles(P〈M0. 05). The increased value of progesterone levels on the day of dating embryo transfer in LUF group [(6.07± 2. 71) vs. (8. 57 ± 2.71) nmol/L] was significantly lower than that in normal ovulation cycles (P 〈0. 05). The embryo implantation rate (11.56% vs. 22.85%)and clinical pregnancy rate (18.84% vs. 35. 99)in LUF group were significantly lower than those in normal ovulation cycles(P〈0.05). Conclusions. The progesterone levels are lower and increase slowly,and the embryo implantation rate and the clinical pregnancy rate are lower in LUF cycle compared with normal ovulation cycles. It would influence embryo implantation to cause failure pregnancy. The FET should be cancelled in LUF cycle.
