摘 要:目的了解肝癌患者血中微量元素和氨基酸代谢的特点,以期更好地指导临床治疗。方法选取2013年3-11月于河北省唐山市妇幼保健院和唐山市古治区医院未经治疗的125例原发性肝癌患者,另选取河北省唐山市妇幼保健院50例健康体检者作为对照。用原子吸收光谱仪对所有研究对象进行血微量元素检测并用氨基酸分析仪检测氨基酸浓度。计量资料组间比较采用t检验。结果肝癌患者血中铜水平显著高于正常对照组,差异具有统计学意义(t=6.30,P<0.05),锌、铁、镁水平均显著低于正常对照组,差异具有统计学意义(t值分别为10.15、3.98、2.25,P值均<0.05);氨基酸谱变化显示,肝癌患者中有7种氨基酸发生显著变化,其中酪氨酸、苯丙氨酸和蛋氨酸浓度升高,缬氨酸、异亮氨酸、亮氨酸和精氨酸浓度降低,差异具有统计学意义(t值分别为10.10、13.46、2.34、3.65、2.98、5.67、8.35,P值均<0.05)。结论肝癌患者血中存在多种氨基酸和微量元素浓度的改变,检测微量元素和氨基酸对于了解肝癌患者病情及指导临床治疗具有一定的意义。Objective To investigate the characteristics of trace elements and amino acid metabolism in the blood of patients with hepatocellular carcinoma( HCC) and to guide clinical treatment. Methods This study included 125 untreated patients with primary HCC in Tangshan Maternal and Child Health Hospital and Tangshan Guzhi District Hospital from March to November,2013,as well as 50 healthy controls who underwent physical examination in Tangshan Maternal and Child Health Hospital. Blood trace elements were measured by atomic absorption spectrophotometry,and the levels of amino acids were determined by an amino acid analyzer. Comparison of continuous data was made by t test. Results Compared with the controls,the HCC patients had a significantly higher Cu level( t = 6. 30,P〈0. 05) and significantly lower Zn,Fe,and Mg levels( t = 10. 15,3. 98,and 2. 25,all P〈0. 05). The HCC patients showed significant increases in tyrosine,phenylalanine,and methionine levels( t = 10. 10,13. 46,and 2. 34,all P〈0. 05) and significant decreases in valine,isoleucine,leucine,and arginine levels( t = 3. 65,2. 98,5. 67,and 8. 35,all P〈0. 05). Conclusion HCC patients show changes in various amino acids and trace elements in blood. Measurement of trace elements and amino acids is of certain significance for evaluating HCC patients' condition and guiding clinical treatment.
