磁敏感加权成像对急性脑梗死动脉血栓的诊断价值

摘　　要：目的:探讨磁敏感加权成像(SWI)技术对急性脑梗死动脉血栓的诊断价值。材料与方法:20例急性期脑梗死患者均在3.0T MR成像系统上行常规MRI、DWI、MRA和SWI序列。采用最小密度投影(mIP)观察颅内动脉血栓情况。结果:20例急性脑梗死病变中MRA显示有8例发生一侧大脑中动脉闭塞,9例大脑中动脉呈不同程度狭窄,3例MRA未见异常。急性梗死灶均位于患侧责任血管供血范围内。在20例患者中,有11例在SWI序列显示大脑中动脉内血栓,其中10例发生在大脑中动脉水平段或分支起始部,1例发生在大脑中动脉分支远端,与MRA图像动脉阻断区位置大致吻合。SWI对血栓的显示阳性率为55%。在正常血管及颅内狭窄的非责任血管内SWI均未显示动脉血栓。结论:SWI技术对急性脑梗死动脉血栓的诊断和指导治疗中具有重要的价值,可作为检测急性脑梗死血栓存在的主要检查序列。Purpose： To evaluate the value of susceptibility weighted imaging（SWI） in diagnosing arterial thrombosis in acute cerebral infarction. Materials and Methods： Twenty patients with acute cerebral infarction were studied using a 3.0TMR scanner. All patients underwent MR imaging with the following sequence ： conventional MRI, DWI, MRAand SWI. The SWI images were reconstructed to display the cerebral arterial thrombosis by the minimum intensity projection（mIP）. Re- suits： Among twenty cases with acute infarction, MRA showed obstruction of middle cerebral artery （MCA）in 8 cases and ste- nosis in varying degrees of MCA in 9 cases. 3 cases of MRA were normal. The acute infarcts were located in the area supplied by the responsibility of vascular in the lesion side. In 20 cases, llcases showed MCA thrombosis on SWI. 10 of 11 cases oc- curred in the level or branch starting segments of MCA, and 1 case occurred in distal branch of MCA. The location of throm- bosis on SWI consisted with the broken region of MRA. The positive rate of thrombosis showed on SWI was 55%. There was no arterial thrombosis showed on SWI in normal vascular and the non-responsibility stenosis artery. Conclusion： SWI is useful for the diagnosis in arterial thrombosis and guide to treatment in the acute cerebral infarction. Therefore SWI should be a main examination for the diagnosis of arterial thrombosis in acute cerebral infarction.
