摘 要:目的探讨不同吸氧浓度对心脏骤停(CA)的大鼠心肺复苏(CPR)后神经功能损伤及脑细胞线粒体能量代谢的影响。方法将40只sD大鼠按照随机数字表法分为假手术组,CA/CPR后100%吸氧浓度(100%O2CA/CPR)组、75%吸氧浓度(75%O2CA/CPR)组、50%吸氧浓度(50%O2CA/CPR)组、21%吸氧浓度(21%O2CA/CPR)组五组,每组8只。假手术组大鼠只行气管插管操作,不进行CA;不同吸氧浓度组大鼠麻醉后心电监护、气管插管并接动物呼吸机,于呼气末夹闭气管,窒息致CA,立即开放呼吸机,各组给予不同浓度的氧气吸入,并结合胸外按压及肾上腺注入使大鼠恢复自主呼吸循环,停止CPR。对所有大鼠按照Longa标准进行神经功能评分;采用高效液相色谱一紫外检测法测量脑组织ATP的含量;用分光光度法检测线粒体呼吸链复合体Ⅲ态、Ⅳ态的活性。结果与对照组比较,21%O2CA/CPR组、100%O2CA/CPR组大鼠神经功能评分显著降低,脑组织ATP含量降低(P〈0.05),呼吸Ⅲ态速率降低(P〈0.05),呼吸Ⅳ态速率加快(P〈0.05)。结论CPR过程中,低浓度氧和高浓度氧均不利于CA后大鼠神经功能恢复及脑细胞线粒体能量代谢。Objective To investigate the influence of difference oxygen concentration on brain function and brain mitochondrial energy metabolism after cardiopulmonary resuscitation in rats. Methods Forty rats were randomly divided into 5 groups: sham group, 100% O2 CA/CPR group, 75% O2 CA/ CPR group, 50%O2 CA/CPR group and 21%O2 CA/CPR group, 8 rats in each group. The sham group only took tracheal intubation, without cardiac arrest. The other groups anesthesia, connect cardiac monitor, tracheal intubation and connect the animal breathing machine in turn. Occlusion the tracheal at end - expiratory, make asphyxia into cardiac arrest, open the breathing machine, inhale different oxygen concentration, chest compressions and adrenaline injection to make spontaneous circulation return, stop CPR. All rats were conducted nerve function evaluation by the score of nerve behavior of NDS. ATP of cerebral tissue was measured by high performance hquid chromatography method. Spectrophotometry was used to detect the mitochondrial respiratory chain complexes Ⅲ state, Ⅳ state activity. Results Compared with the sham group, the nerve function scores and brain ATP content in 21% O2 CA/CPR group and 100% O2 CA/CPR group were remarkably reduced ( P 〈 0.05 ), and the State 3 respiration rate significantly decreased (P 〈 0.05 ), State 4 respiration rate significantly accelerated ( P 〈 0. 05 ). Conclusion In the course of CPR, neither low oxygen concentration and high concentration has beneficial on brain function and brain mitochondrial energy metabolism after cardiopulmonary resuscitation.
