基于全科医生签约服务的全科助理员模式探讨

摘　　要：目的探讨基于全科医生签约服务的全科助理员模式的效果。方法由在社区工作时间较长、沟通能力强、社区护理工作经验丰富的护师作为全科助理员,与全科医生团队形成配比,协助全科医生完成全人群覆盖式的家庭医生责任制服务工作,主要包括建档管理、门诊预约、双向转诊、健康教育等内容。结果全科助理员模式试点后一年与试点前一年相比,全科医生人均累计签约人数增加31.33%,预约门诊人次增加29.02%,新发现高血压患者数增加34.31%,双向转诊人次增加117.86%。全科助理员模式试点运行一年间,匹配助理员与未匹配助理员的全科医生相比,人均累计签约人数增加46.38%,预约门诊人次增加141.41%,新发现高血压患者数增加229.64%,双向转诊人次增加179.82%。结论基于全科医生签约服务的全科助理员模式有利于提高慢性病管控效率,有利于提升社区基本公共卫生覆盖水平,是提升社区人力资源配置效能的有效途径。Objective To investigate the general practice assistant mode based on general practitioner contract-signing service. Methods The nurse practitioners who had longer working years in community,good communication skills and rich experience in community nursing were chosen as the general practice assistants matched with the number of general practitioners. They assisted GPs with the family doctor service under complete coverage of a population,like records management,outpatient service appointment,two- way referral and health education. Results Compared with the previous year,the total GP contract- signing residents increased by 31. 33%,residents who made outpatient service appointments increased by 29. 02%,newly diagnosed hypertensive residents grew by 34. 31% and residents undergoing two- way referral increased by 117. 86%.During the year of GP assistant trail,compared with GPs without assistants,GPs with assistants saw an increase by 46. 38% in the number of contract- signing residents per GP,141. 41% in the number of outpatient service appointments,229. 64% in the number of newly diagnosed hypertensive patients and 179. 82% in the number of patients undergoing two- way referral. Conclusion The general practice assistant mode based on general practitioner contract- signing service helps to improve the efficiency of the management and control of chronic diseases, enhance the level of basic public health coverage. It is an effective way to improve the effectiveness of human resources allocation in communities.
