疏血通注射液合并用药致上消化道出血的疑似病例分析

摘　　要：目的对疑似因疏血通注射液用药期间致上消化道出血患者的病历资料进行分析,为临床合理用药提供参考。方法回顾性分析海南医学院附属医院2010年1月—2013年6月1 876份临床应用疏血通注射液的住院患者的病历资料,其中4例在疏血通注射液用药期间出现上消化道出血,分析其出血与应用疏血通注射液的相关性。结果发生上消化道出血的4例患者均〉70岁;4例患者与疏血通注射液合并应用的药物除阿司匹林肠溶片外,还包括参芎葡萄糖注射液、芪蛭通络胶囊、红花黄色素注射液等抗栓药物,合并用药3~6 d后出现黑便、腹胀、腹痛、呕血等上消化道出血症状,停药和抑酸治疗4~9 d后出血症状好转或痊愈。结论疏血通注射液合并用药可能引起上消化道出血,合并用药导致出血可能与阿司匹林等抗栓药物有关,临床应警惕多种中西抗栓药物的联合应用有导致出血的可能性。Objective To analyze the data of upper gastrointestinal hemorrhage suspected cases that occur during the course of Shuxuetong injection,to provide references for rational drug use.Methods We retrospectively examined the clinical data of 1 876 patients using Shuxuetong injection in the Affiliated Hospital of Hainan Medical University from January 2010 to June2013.Upper gastrointestinal hemorrhage occurred during the shuxuetong injection.The correlation between the gastrointestinal hemorrhage and the application of Shuxuetong injection was analyzed.Results The four patients suffering upper gastrointestinal hemorrhage were all over 70 years old.Apart from aspirin enteric coated tablets,Shuxuetong injection was also combined with Salviae Miltiorrhizae Liguspyragine Hydrochloride and Glucose injection,Qizhitongluo capsule,Safflower yellow injection and other antithrombotic drugs.Three to six days after the administration of the combined drugs,symptoms of upper gastrointestinal hemorrhage appeared,including black stool,abdominal distension,abdominal pain,hematemesis,etc.The symptoms reduced or recovered four to nine days after the drug withdrawal and the use of acid inhibitors.Conclusion Shuxuetong injetion combined with other medications may cause upper gastrointestinal hemorrhage.Drug combination may be related with antithrombotic drugs,like aspirin.Combined application of Chinese and western antithrombotic drugs should be carefully conducted in clinical practice.
