摘 要:目的通过观察帕金森病痴呆(PDD)患者外周血血小板β淀粉样前体蛋白免疫反应片段之比(βAPPr)的变化,分析βAPPr与PDD的关系,为PDD诊断提供思路。方法收集2011年4月—2014年4月在郑州大学附属洛阳中心医院神经内科就诊的帕金森病(PD)患者124例,按是否合并痴呆分为PDD组59例和对照组65例。评价简易智能量表(MMSE)评分、蒙特利尔认知评估量表(Mo CA)评分、临床痴呆评定量表(CDR)评分、统一帕金森病评定量表(UPDRS)评分;采用Western blotting法检测外周血血小板βAPPr。结果 PDD组患者受教育年限、MMSE评分、Mo CA评分低于对照组,CDR评分、UPDRSⅠ评分高于对照组(P<0.05)。PDD组患者外周血血小板βAPPr(5.43±0.92)低于对照组的(7.88±1.32)(t=6.28,P<0.001)。PDD组患者βAPPr与Mo CA评分中视空间与执行能力、命名能力、注意能力、延迟回忆能力、定向力得分均呈正相关(P<0.05)。结论 PDD患者血小板βAPPr较PD患者下降,血小板βAPPr有可能成为PDD诊断的外周血生物学指标之一,外周血血小板淀粉样前体蛋白(βAPP)表达水平有可能为研究PDD的发病机制和治疗提供线索。Objective To investigate the relationship between Parkinson's disease with dementia (PDD) and β - amyloid precursor protein forms ratio (βAPPr) in peripheral platelets by the observation of the changes in β APPr in the peripheral platelets of patients with PDD, in order to provide new insights for the diagnosis of PDD. Methods 124 patients with PDD who received treatment in the Department of Neurology of Central Hospital Affiliated to Zhengzhou University from April 2011 to April 2014. According to whether dementia occurred, the subjects were divided into two groups: PDD group (n = 59) and control group (n = 65) . All the patients were evaluated by using Mini - mental state examination (MMSE), Montreal Cognitive Assessment (MoCA), Clinical Dementia Rating (CDR) and Unified Parkinson's Disease Rating Scale (UPDRS) ; βAPPr of the two groups was determined by Western blotting method. Results PDD group was lower ( P 〈 0. 05 ) than control gorup in education years, MMSE score and MoCA score and higher ( P 〈 0.05 ) than control group in CDR score and UPDRS I score. PDD group was lower than conrol group in I3APPr (5.43 ± 0. 92, 7. 88 ±1.32 ; t = 6. 28, P 〈 0. 001 ) . In PDD group, βAPPr was positively correlated ( P 〈 0.05 ) with the scores of several MoCA items, including visuospatial ability, executive capabihty, naming ability, attention, delayed recall ability, and orientation ability. Conclusion PDD patients have lower βAPPr than PD patients, which means βAPPr in platelets could be used as one of the peripheral biological indicators for the diagnosis of PDD. β APP elements in peripheral platelets may also provide new insights into the pathological mechanism and treatment of PDD.
