摘  要:目的研究HBV基因分型和耐药突变基因的特点及核苷(酸)类似物耐药突变情况。方法收集HBV患者血清141例,PCR点杂交法检测HBV基因分型和耐药突变基因。结果 105例HBV DNA阳性患者中,B基因型占62.86%,C基因型占37.14%,差异有统计学意义(P<0.05);B基因型和C基因型的耐药突变率分别为31.82%和30.77%,差异有统计学意义(P<0.05)。B基因型以204I和180M+204V+204I突变多见。HBV耐药突变模式以rt204I(11.43%)和180M+204V+204I(8.57%)为主。LAM耐药率显著高于ADV,差异有统计学意义(P<0.05),但与Ld T+ETV耐药率差异无统计学意义(P>0.05)。B基因型和C基因型耐药均以LAM+Ld T+ETV为优势模式。结论本地区HBV基因B型多于基因C型,204I和180M+204V+204I基因突变模式多见,LAM和LAM+Ld T+ETV耐药模式多见。Objective To investigate the characteristic of HBV genotyping, drug resistant mutation, and the resistance mutation to nucleoside/nucleotide analogues. Methods Serum of 141 patients with chronic HBV was collected, PCR - spot hybridization was used to detect the Hepatitis B Virus genotyping and the drug resistant mutation. Results The rate of genotype B was 62.86% in 105 HBV DNA positive patients, and genotype C was 37.14%, and the difference had statistical significance(P 〈 0.05). The drug resistant mutation rates in genotype B was 31.82% and 30.77% in genotype C, and the difference had statistical significance (P 〈 0.05 ). 204I and 180M + 204V + 204I mutation were the main in genotype B ; rt204I ( 11.43% ) and 180M + 204V + 204I(8.57% ) mutation were the main type in HBV patients. LAM resistance rate was higher than ADV, and the difference had statistical significance(P 〈 0.05), but there was no statistics significance on the difference with LdT + ETV (P 〉 0.05). LAM + LdT + ETV resistance was the superiority both in genotype B and genotype C. Conclusion The number of HBV genotype B was more than genotype C in this locality. 204I and 180M + 204V + 204I mutation were the main. LAM and LAM + LdT + ETV resistance was the superiority.
