食管鳞癌组织中黑色素瘤相关抗原D4b的表达及临床意义

摘　　要：目的:探讨黑色素瘤相关抗原D4b(melanoma associated antigens-D4b,MAGE-D4b)在食管鳞癌(ESCC)组织中的表达及其与临床病理因素之间的关系。方法:采用免疫组织化学法检测75例食管鳞癌石蜡标本和36例正常食管黏膜组织中MAGE-D4b蛋白表达情况。结果:ESCC中MAGE-D4b的表达水平明显高于正常食管黏膜中的表达水平(分别为42.7%和16.7%,P<0.01)。MAGE-D4b蛋白的阳性表达水平与食管鳞癌患者的性别、年龄、肿瘤大小、肿瘤部位、浸润深度、肿瘤分化及AJCC分期等临床病理因素均无相关性(P>0.05)。Kaplan-Meier生存分析显示,食管鳞癌组织中MAGE-D4b阳性表达者的3年生存率低于阴性表达者(3年生存率:38.7%和69.2%,P=0.013)。单因素分析及Cox多因素分析均提示年龄、MAGED4b的表达及淋巴结转移是有意义的预后因素(P<0.05)。结论:MAGE-D4b蛋白在食管鳞癌组织中高表达,且其阳性表达与患者的3年生存率相关,MAGE-D4b蛋白可能为食管鳞癌的预后判断提供有意义的生物学指标。Objective： To investigate the expression of MAGE-D4 b protein and it＇ s relationship with clinical pathological parameters of esophageal squamous cell carcinoma（ ESCC）. Methods： Expression of MAGE-D4 b protein was detected by immunohistochemistry in tissue samples from 75 ESCC cases and 36 healthy control cases. Results： The expression of MAGE-D4 b protein in the ESCC was significantly increased compared with that in the normal esophageal tissue（ 42. 7% and 16. 7%,respectively,P〈 0. 01）. The expression of MAGE-D4 b was not related to gender,age,tumor size,tumor location,grading invasive depth,tumor differentiation,lymph node metastasis and clinical AJCC stages（ P〉 0. 05）. There was statistically correlation between positive expression of MAGE-D4 b protein and 3 years survival by Kaplan-Meier analysis（ 3-year survival： 38. 7% and 69. 2%,P = 0. 013）. Univariate analysis and Cox multiple factors analysis identified age,the expression of MAGE-D4 b protein and lymph node metastasis was siginificant prognostic factors（ P〈 0. 05）. Conclusion： MAGE-D4 b protein was highly expressed in ESCC. The expression of MAGE-D4 b protein was correlated with 3 years survival time,MAGE-D4 b protein maybe a significant biological index of prognosis in ESCC.
