复方苦参注射液辅助GP方案治疗老年晚期非小细胞肺癌近期疗效及安全性

摘　　要：目的:观察复方苦参注射液联合吉西他滨加顺铂治疗老年晚期非小细胞肺癌的近期疗效及安全性。方法:选取北京军区总医院收治的152例确诊为IIIb-IV期的老年晚期非小细胞肺癌患者,随机分成观察组(n=76)和对照组(n=76)。给予两组相同的化疗方案:第1、8d静脉滴注吉西他滨800mg/m2,顺铂60mg/m2静滴d1-2,21d为1个周期。除此之外给予观察组联合复方苦参注射液15ml,静脉滴注,1次/d,d1-14,21天重复。治疗2周期后分析两组患者肿瘤控制情况及化疗不良反应。结果:观察组有效率(CR+PR)为42.11%(32/76),疾病控制率(CR+PR+SD)为68.42%(52/76);对照组有效率(CR+PR)为40.79%(31/76),疾病控制率(CR+PR+SD)为64.47%(49/76),差异无统计学意义,但观察组的有效率和疾病控制率略好。观察组化疗Ⅲ-Ⅳ度不良反应发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:复方苦参注射液联合吉西他滨加顺铂化疗治疗老年晚期非小细胞肺癌效果好,安全可靠,适合在临床推广应用。Objective： To observe the short-term clinical efficacy and safety of compound matrine injection combined with gemcitabine in the treatment of elderly patients with advanced non-small cell lung cancer. Methods： All152 elderly cases with stage IIIb-IV non-small cell lung cancer diagnosed in General Hospital of Beijing Military Area Command were randomly divided into observation group and control group,76 cases in each. The two groups were given the same chemotherapy： Gemcitabine 800 mg / m^2 by IV drip on day 1 and day 8. Cisplatin 60 mg / m^2 by IV drip on day 1 and day 2 of each 21-day cycle. Additionally,the observation group was given compound matrine injection,15 ml / d,q. d,by IV drip on day 1 and day 14 of each 21-day cycle. After two cycles of treatment,the tumor control and the side effects of chemotherapy in the two groups were compared. Results： In the observation group,effective rate（ CR ＋ PR） was 42. 11%（ 32 /76）,and disease control rate（ CR ＋ PR ＋ SD） was 68. 42%（ 58 /76）. In the control group,effective rate（ CR ＋ PR） was 40. 79%（ 31 /76）,and disease control rate（ CR ＋ PR ＋ SD） 64. 47%（ 49 /76）. Both the effective rate and the disease control rate in the observation group were slightly better,though the difference between the two groups was not statistically significant. However,the grade III-IV adverse reaction rate of chemotherapy in the observation group was significantly lower than that of the control group and the difference was statistically significant（ P〈 0. 05）. Conclusion： Compound matrine injection combined with gemcitabine and cisplatin chemotherapy in treatment of elderly patients with advanced non-small cell lung cancer is effective and safe.
