牙周塞治剂对拔牙创周围组织影响的实验研究

摘　　要：目的 观察牙周塞治剂对拔牙创周围组织的影响.方法 本研究于2013年1月至2014年12月在南京军区福州总院比较医学科和福建医科大学口腔医学院中心实验室完成.60只3～4月龄SD雄性大鼠随机分为A、B两组,A组36只,B组24只.所有大鼠分别拔除双侧上颌第一磨牙,A组大鼠一侧创口不处理（A1组）,另一侧创口敷牙周塞治剂（A2组）;B组大鼠一侧创口填塞明胶海绵（B1组）,另一例创口填塞明胶海绵后敷牙周塞治剂（B2组）.观察各组创口止血时间;于治疗后1、3、5、7d后两组大鼠各处死6只,10、14d后再处死A组大鼠各6只,观察拔牙创愈合情况;取上颌骨标本做病理切片观察,观察拔牙创周围组织的炎症情况.结果 A1组创口出血时间较其他3组（A2、B1、B2组）时间长.所有大鼠创口愈合良好,未见明显感染.病理观察结果：1、3和5d后所有的大鼠创口周围均有炎症细胞浸润,7、10和14d后基本来见炎症细胞;3d后,A2组炎症重于其余3组（A1、B1、B2组）;7d后牙周塞治剂未脱落的创口炎症细胞浸润较已脱落创口明显.结论 拔牙创敷牙周塞治剂后能快速止血,7d内不影响创口愈合,对拔牙创周围组织几无影响.Objective To evaluate the effect of periodontal dressings on the surrounding tissues of tooth sockets. Methods Sixty male SD rats were removed bilateral maxillary first molars, and randomly divided into two groups as follows ： A： non-gelatin sponge （n=36）, one side was treated by periodontal dressings（A2） and the other side not treated（A1 ） ; B： gelatin sponge （n=24）, one side is filled with gelatin sponge （B1）, and the other side was treated with periodontal dressings after being filled with gelatin sponge （B2）. Histological evaluation was assessed about the intensity of inflam- matory response at postoperative periods of 1, 3, 5,7,10 and 14 days. Results Bleeding time of group A1 was longer than the other groups. The inflammatory ceils were infiltrated at 1, 3 and 5 days, and no inflammatory cells at 7, 10 and 14 days, but there were still many inflammatory cells infihration when periodontal dressings existed. At 3 days, there were more inflammatory ceils in A2 group than other groups. Seven days later, there were still inflammatory cells in sockets with periodontal dressings, while none in those without periodontal dressings. Conclusion Periodontal dressings can promote of rapid hemostasis tooth sockets and have no adverse effect on the wound in short time.
