摘 要:目的探讨老年2型糖尿病合并恶性肿瘤患者临床特征。方法采用回顾性队列研究的方式,连续收集2010年1月-2014年12月成都军区总医院收治的2771例老年2型糖尿病住院患者临床资料,分析恶性肿瘤的分布情况。结果恶性肿瘤检出率为6.8%(189/2771)。根据是否合并恶性肿瘤分组,恶性肿瘤组男性比例高于无恶性肿瘤组(58.2%vs.48.9%,P〈0.01);恶性肿瘤组平均年龄(77.3±8.9)岁,高于无恶性肿瘤组的(72.8±11.6)岁,差异有统计学意义(P〈0.01)。恶性肿瘤分布:消化系统90例(47.6%)、呼吸系统39例(20.6%)、血液系统12例(6.3%)、女性生殖系统35例(18.5%)、男性泌尿生殖系统7例(3.7%)、其他类别肿瘤6例(3.2%);前5位的疾病构成分别为:结直肠癌(16.4%)、肺癌(15.3%)、肝癌(14.3%)、乳腺癌(7.4%)、胰腺癌(5.3%)/子宫内膜癌(5.3%)。单因素分析显示:性别、年龄、吸烟、饮茶、糖尿病家族史为老年2型糖尿病患者并发恶性肿瘤的影响因素(P〈0.05)。结论2型糖尿病患者合并恶性肿瘤消化道肿瘤最为常见,性别、年龄、吸烟、饮茶、糖尿病家族史是并发恶性肿瘤的影响因素。Objective To Analysis the cancer distribution of hospitalized type 2 diabetic pa- tients, and to explore the risk factors of type 2 diabetes mellitus patients concomitant to cancer. Methods A retrospective cohort study was adopted to collect 2 771 cases of type 2 diabetic patients admitted to the department of cadres ward of General Hospital of Chengdu Military of PLA from January 2010 to December 2014. Clinical data of type 2 diabetes patients were collected for analysis of distribu- tion of the malignancies, Univariate analysis was used in screening risk factors of type 2 diabetes mellitus concomitant to cancer. Results A total of 189 (6.8%) as malignant group were detected with cancer. The proportion of men was significantly higher in malignant group than that in patients without cancer group (58.2% vs. 48.9% ,χ2 = 11. 855 ,P 〈0.01 ), the average age of malignant group was higher than those without cancer [ (77.3 ±8.9)years vs. (72.8 ± 11.6) years,t =5. 222,P 〈0.011. Out of 189 pa- tients with cancer, 90 cases in digestive system (47.0%), 39 cases with respiratory system (20.6%), 12 cases with blood system (6.3%), 35 cases with female reproductive system ( 18.5% ), 7 cases with male genitourinary system (3.7%), other types of tumors 6 cases (3. 6% ). The top five cancer observed were: colorectal cancer ( 16.4% ), lung cancer ( 15.3% ), liver cancer ( 14. 3% ), breast cancer (7.4%), pancreatic cancer (5.3%)/endometrial cancer (5.3%). Univariate analysis showed that gender, age, smoking, drinking, family history of diabetes were influencing factors for type 2 diabetes patients suffering malignant tumors. Conclusion The most common cancer in type 2 diabetic patients was gastrointestinal cancer. Gender, age, smoking, drinking, family history of diabetes are potentially influencing factors of type 2 diabetes patients suffering malignant tumors.
