不同液体垫对大肠息肉内镜下黏膜切除术并发症的影响

摘　　要：目的探讨不同成份液体垫对大肠息肉内镜下黏膜切除术后并发症的影响。方法350例息肉患者分为生理盐水组176例和肾上腺素组174例。内镜黏膜切除术前，生理盐水组在息肉基底部黏膜下层单纯注射生理盐水，注射后见病变抬起，再予圈套切除。肾上腺素注射液为1：10000肾上腺素生理盐水混合液。比较2组息肉术后出并发症发生差异。结果肾上腺素组术后即刻出血率、迟发性出血率均显著低于生理盐水组（P〈0．05），2组均无穿孔发生。结论大肠息肉内镜下黏膜切除术中，使用1：10000肾上腺素生理盐水溶液作为液体垫能更有效降低术后出血并发症的发生风险。Objective To investigate the effect of different liquid pads in reducing risk of complications after colorectal polyp endoscopic mucosal resection （EMR）. Methods A Total of 350 patients with colorectal polyp were randomly divided into normal saline group（ n = 176） and epineph- rine group（n = 174）. In the normal saline group, saline was injected alone in the base of the polyp submucosa, making the lesion lift. In the epinephrine group, adrenaline saline mixed solution with a ratio of 1 ： 100 00 was injected instead of saline alone. The difference in incidence of postoperative bleeding and perforation complications was compared between the two groups. Results The immedi- ate rate and delayed bleeding rate in the epinephrine group were significantly lower than that of the sa- line group （P 〈 0.05 ）. There was no perforation in either group. Conclusion Using 1 ： 100 00 adrenaline saline solution as a liquid pad in EMR of colorectal polyps can reduce the risk of postopera- tive bleeding complication more effectively than using saline alone.
