经透明角膜切口25G玻璃体切割系统在婴幼儿先天性白内障中的应用

摘　　要：目的:评价经透明角膜缘切口25G玻璃体切割手术系统(TSV25G)在婴幼儿先天性白内障手术中应用的临床疗效。方法:回顾分析17例(28只眼)婴幼儿先天性白内障患者的临床资料。其中5例(8只眼)为早产儿,在行早产儿视网膜疾病筛查时发现先天性白内障。12例(20只眼)为足月产婴幼儿,因家长发现其白瞳症而就诊。所有患儿在全身麻醉下通过25G玻切头经透明角膜切口行晶状体切除、后囊膜环形切开以及前部玻璃体切割手术。手术后随访3个月至1年,观察术后视力、眼压、前房反应、虹膜后粘连、后囊膜混浊等术后并发症情况。结果:17例(28只眼)术后视力较术前均有提高。术后前房反应轻,无虹膜后粘连、后囊膜混浊、低眼压、眼内炎、黄斑囊样水肿和视网膜脱离等并发症发生。结论:对于婴幼儿先天性白内障,通过25G玻切头经透明角膜切口行晶状体切除、后囊膜环形切开以及前部玻璃体切割手术,具有操作容易控制、手术创伤小、术后炎性反应轻等优点,能有效抑制后发性白内障的发生,有助于视功能的恢复。Objective To evaluate the curative efficacy of 25-gauge（ 25G） transcornea sutureless vitrectomy system（ TSV25G） on the treatment of infant with congenital cataract. Methods The clinical data of 17 infants including 28 eyes with congenital cataract were retrospectively analyzed,in which 5 premature infants including 8 affected eyes were found in retinal diseases screening,and another 12 term birth infants including 20 affected eyes were found by their parents because of leakocoria. All infants underwent lensectomy,posterior peplos circular cut and anterior vitrectomy by TSV25 G under general anaesthesia,and followed up for 3 months to 1 year. The postoperative vision,intraocular tension,anterior chamber response were observed,and the complications such as irisposterior synechiae,posterior peplos opacification were recorded.Results The vision of 17 infants including 28 eyes were all improved postoperation. The anterior chamber responses were slight,and no complications such as irisposterior synechiae,posterior peplos opacification,low intraocular tension,endophthalmitis,macular vesicular edema and retinal detachment were found in all cases. Conclusion The advantages of 25 G transcornea sutureless vitrectomy system in treatment of infants with congenital cataract are easy to operate,less operation wound,slight inflammatory reaction,and the after-cataract could be effectively inhibited,suggesting that it is helpful for visual function recovery.
