摘　　要：目的探讨成人肝母细胞瘤(HB)的CT表现。方法回顾性分析9例经手术病理证实的成人HB患者资料,8例患者行CT平扫和动态增强扫描检查,1例行CT平扫检查。分析其影像特征。结果肿瘤位于肝右叶6例,肝左叶2例,累及肝左右叶1例。单发8例,多发1例。9例肿瘤密度不均,均可见坏死囊变,5例伴有出血,2例伴有钙化,7例肿瘤有假包膜。所有肿瘤均未见肝硬化、血管侵犯、肝内胆管扩张及周围卫星灶征象,1例伴有肝门及腹膜后淋巴结转移。CT增强扫描肿瘤区实性成分及纤维间隔呈不均匀渐进性强化,强化程度低于周围正常肝组织,出血、坏死、囊变区未见明显强化。结论成人HB在CT图像上具有一定的特征性表现,再结合临床特点,有助于提高该病的诊断准确率。Objective To investigate the imaging findings of Hepatoblastoma in an adult with dynamic spiral CT.Methods Nine cases of Hepatoblastoma in an adult were collected.All cases were confirmed by operation and pathology.Both Pre-contrast and post-contrast scans with spiral CT were performed in 8 cases.Plain scons with spiral CT was performed in 1 case.Imaging findings in all cases were retrospectively reviewed.Results Tumors located in 6 cases in the right liver lobe,left hepatic lobe in 2 cases,involvement of the liver lobe 1 case.8 cases had Single nodule,1 casehad multiple nodules.Uneven density,9 cases of patients with tumor necrosis sac were visible,5 cases with hemorrhage,2 cases with calcification,7 cases of patients with the rim enhancement of tumor capsule.In all tumors liver cirrhosis,metastasis to blood vessel,channel of inside liver extend,and lymph node elephantiasis were not seen,only 1 case had portal and retroperitoneal lymph node metastasis.CT enhanced scan tumor area solid components and fiber interval had non-uniform progressive reinforcement,reinforcement degree was lower than the normal liver tissue around; hemorrhage,necrosis,cystic areas were not seen to have obvious reinforcement.Conclusion The imaging features of Hepatoblastoma in an adult on plain and enchanced spiral CT combined with clinical characteristics can give a clue on diagnosis and increase diagnostic accuracy.
