摘 要:目的研究超声造影在肝门部胆管癌术前评估中的临床价值,丰富临床治疗的经验。方法 88例经手术病理证实的肝门部胆管癌患者,采用常规超声与超声造影进行检查,比较回声分布差异。结果在超声造影的门脉期与延迟期分别有77.3%(68/88)和87.5%(77/88)的肝门部胆管癌表现为低回声,且与周围肝实质分界较清晰;而常规超声53.4%(47/88)表现为等回声,与周围肝实质分界不清晰,常规超声与门脉期、延迟期的病灶显示清晰率分别为46.6%(41/88)、79.5%(70/88)、87.5%(77/88),清晰率比较差异具有统计学意义(P<0.05)。结论在肝门部胆管癌术前评估中,超声造影具有准确率高的应用价值,值得在临床中推广应用。Objective To research clinical value of ultrasound contrast in preoperative evaluation of hilar cholangiocarcinoma, in order to provide abundant clinical treatment experience. Methods There were 88 hilar cholangiocarcinoma patients under operational pathology identification. They received conventional ultrasound and ultrasound contrast for examination, and comparison was made on difference of echo distribution. Results In portal phase and delayed phase of ultrasound contrast, there were 77.3%(68/88) and 87.5%(77/88) of hilar cholangiocarcinoma shown as hypoecho, with clear boundary with surrounding liver parenchyma. There were 53.4%(47/88) by conventional ultrasound shown as equal echo, without clear boundary with surrounding liver parenchyma. The clear rate of lesions by conventional ultrasound, portal phase and delayed phase were respectively 46.6%(41/88), 79.5%(70/88), 87.5%(77/88). Their clear rates had statistically significant difference(P〈0.05). Conclusion Ultrasound contrast contains application value as high accuracy in preoperative evaluation of hilar cholangiocarcinoma, and this method is worthy of clinical promotion and application.
