摘  要:目的探讨双能量CT碘定量法对兔肝纤维化评价的应用价值。方法对55例新西兰兔先行造模、CT扫描、病检分期。图像后处理避开血管与胆道,分别选取感兴趣区,测量肝实质平扫、门脉期、实质期的碘值,定量分析肝纤维化的严重程度。各肝纤维化级别组碘值结果比较采用方差分析,碘值与肝纤维化严重程度的相关性采用Spearman法。结果随着肝纤维化程度的加重,三期时相的碘值呈现出下降的趋势,且门脉期S3、S4期分别与S0、S1、S2期的差异有统计学意义。门脉期碘值与肝纤维化严重程度的相关性较高(r=-0.815,P<0.05)。结论双能量CT碘定量法能定量分析肝纤维化病程中的改变,尤其是门脉期的碘值与肝纤维化严重程度的相关性较高,对临床诊断及治疗后的随访具有较高的应用价值。Objective The goal of this study was to estimate the clinical value of dual-energy CT iodine imaging in evalu- ating the stage of liver fibrosis. Methods Fifty-five healthy new zealand white rabbits were selected to set up the animal model and then underwent CT scanning andthe pathological examination to determine the stage. The image post-processing should be needed to avoid vascular and biliary. The ROI of the liver parenchyma was selected andthe iodine values in dif- ferent scannings at arterial phase, portal venous phase, and parenchymal phase were recorded. The iodine results of differ- ent severity of fibrosis were compared with one way ANOVA test. The correlation of iodine value and severity of fibrosis were performed with spearman method. Results The odine values in three phases decreased with the increase severity of liver fibrosis and the differences of iodine value between SO, S1 stage to S2, S3, S4 stage were significantly different ( P 〈0.05). The iodine value was shown little relationship with the severity of fibrosis ( r =-0. 815, P 〈0.05). Conclusion As a new CT technique, the dual-energy CT iodine imaging can quantitatively evaluate the course of liver fibrosis and, es- pecially the odine value in portal venous phase, which shows a great relationship with the severity of fibrosis, possesses a high value in the clinical diagnosis and post-treatment follow-up.
