不同康复方案治疗脑卒中后偏瘫患者的临床疗效

摘　　要：目的观察综合康复疗法与分期取穴法结合康复训练治疗法对脑卒中后偏瘫患者的临床疗效.方法选取荆州市第一人民医院2012年11月至2013年11月脑卒中偏瘫108例患者的临床资料,随机分为运动疗法结合作业疗法康复组(A组),综合康复组(B组),分期取穴法组(C组)各36例,A组只接受运动疗法(PT)+作业疗法(OT)治疗;B组接受综合康复治疗药物+PT+OT+认知功能训练+心理康复+功能性电刺激+脑神经促通仪改善脑循环;C组使用康复训练结合分期取穴法治疗,观察治疗前后的相关数据并予以分析.结果 3组患者治疗前后的FMA与LOTCA评分的差值进行比较,差异有统计学意义(P<0.05);3组患者治疗前后差值之间的比较,C组优于对照A组、B组,且差异具有统计学意义(P<0.05).结论分期取穴法与康复训练相结合的治疗方式在脑卒中后偏瘫中的临床效果最为显著,值得在临床上推广.Objective To observe the clinical curative effect of comprehensive rehabilitation therapy and stage point selection method combined with rehabilitation therapy for patients with hemiplegia after stroke. Methods 108 patients were randomly chosen in self institute in November 2012 - November 2013 with cerebral apoplexy hemiplegia. They were randomly divided into kinetotherapy combined with occupational therapy group （group A） , the comprehensive rehabilitation group （group B）, and stage point selection method group （group C）, with each of 36 cases. Group A only accepted exercise therapy （PT） ＋ occupational therapy （OT） treatment, group B received comprehensive rehabilitation treatment ＋ PT ＋ OT ＋cognitive function training ＋ functional electrical stimulation ＋ promote flow meter of cranial nerve to improve cerebral circulation, and group C used the method of rehabilitation training combined with stage point selection method treatment. The related data before and after treatment were observed and analyzed. Results The differences in FMA and LOTCA scores of patients among three groups before and after treatment were statistically significant （P 〈 0.05） . The difference of group C was better than that of groups A and B （P 〈 0.05） Conclusion The stage point selection method combined with rehabilitation training has the most significant effect on the treatment of cerebral apoplexy hemiplegia, is worth promoting in clinic.
