内镜下尼龙套扎圈并高频电切治疗消化道大息肉疗效观察

摘　　要：目的：探讨内镜下尼龙圈联合高频电切治疗消化道大息肉的临床疗效和安全性。方法选择2009年1月至2011年12月经胃肠镜检查确诊的有蒂或亚蒂消化道大息肉（直径≥2cm）86例（共91枚）。对于有蒂息肉先用尼龙圈经内镜套扎于息肉根部，再予高频电切术切除；对于亚蒂息肉先予黏膜下注射生理盐水，然后用尼龙圈经内镜套扎于息肉根部，最后高频电切术切除。术后3个月、6个月、1年复查，评估其疗效及相关并发症。结果所有息肉均一次性成功套扎并同时行高频电切除术。3例有蒂息肉少许渗血，经原尼龙圈下方再次套扎后出血停止。所有患者均未见迟发性出血、穿孔等并发症，术后复查均未见复发。结论内镜下，尼龙圈联合高频电切治疗消化道大息肉是一种简便、安全、有效的治疗方法。Objecive To evaluate the clinical efficacy and safety of endoscopic nylon endoloop combining with high-frequency electroresection for treating gastrointestinal large polyps.Methods91 gastrointestinal large polyps（≥2cm）in 86 patients were treated by combined treatment of endoscopic nylon endoloop and high-frequency electro-resection. Before high-frequency electro-resection,we ligated the root of long pedunculated polyps using the nylon endoloop. Submucosal injection of normal saline to subpedunculated polyps was used firstly before ligation and electro-resection. All patients were followed-up and were rechecked by endoscope on 3 month,6 month and 12 month after treatment.ResultsAll polyps are one-time ligated and high-frequency electroresected successfully. 3 cases had errhysis after electroresected immediately and stopped bledding after another endoloop ligated below the first nylon endoloop. No complications such as delayed bleeding and perforation occurred. We also have not found palindromia in all patients.ConclusionEndoscopic nylon endoloop combining with high-frequency electroresection therapy is a simple,safe and effective method in the treatment of gastrointestinal large polyps.
