摘  要:目的:系统评价以铂类药物为基础的支气管动脉灌注术(bronchial arterial infusion,BAI)联合125Ⅰ粒子治疗在中晚期肺癌的有效率、生活质量及生存期中的意义。方法:计算机检索Pub Med、EMBASE、Cochrane Library、VIP、CNKI、万方等数据库,检索时间从各数据库建库至2014年12月。纳入以铂类药物为基础的BAI联合125Ⅰ粒子治疗中晚期肺癌的随机对照试验(randomized controlled trial,RCT),由两名评价者按Cochrane系统评价方法独立评价纳入研究的质量并提取资料,并用Rev Man5.0软件进行统计分析。结果:共纳入4篇研究,共507例患者。Meta分析结果显示:联合组患者的治疗总有效率明显高于BAI组(P<0.05);两组患者间的不良反应发生率对比无统计学差异(P>0.05)。结论:与单独BAI治疗相比,以铂类药物为基础的BAI联合125I粒子治疗中晚期肺癌能提高疗效。Objective: To systematically evaluate the significance of cisplatin-based bronchial artery infusion( BAI) with125Ⅰ radioactive seeds implantation in the effective rate,life quality and survival time for the treatment of advanced lung cancer. Method: Literatures on randomized controlled trials( RCT) of cisplatin-based BAI and 125Ⅰ seeds for the treatment of advanced lung cancer were searched in Pub Med,EMBASE,the Cochrane Library,VIP,CNKI and Wan Fang database from the establishment of these databases to December 2014. The independent evaluation about the quality of the data included for research was carried out by two valuators according to the Cochrane system evaluation method,with data selected and statistically analyzed by Rev Man5. 0 software. Results: Four RCTs( 507 patients altogether) were included. The Meta-analysis showed that the clinical total effective rate was significantly higher in the BAI and125 I seeds group than that in BAI alone group( P〈0. 05). The incidence of adverse reactions between the two groups was no statistically significant difference( P〈0. 05). Conclusion: Compared with BAI alone,the combination of cisplatin-based BAI with125 I seeds for the treatment of advanced lung cancer has advantages in improving the curative effect.
