调和化瘀解毒法治疗慢性萎缩性胃炎疗效观察

摘　　要：目的观察调和化瘀解毒法治疗慢性萎缩性胃炎的临床疗效。方法将67例慢性萎缩性胃炎患者随机分为治疗组35例和对照组32例,治疗组根据调和化瘀解毒法制定基本方,随证加减;对照组服用胃复春片和维酶素片。2组疗程均为3个月。观察2组治疗后临床症状、胃镜病理和血液流变学的改善情况。结果治疗组中医证候总有效率明显高于对照组(P<0.05);治疗后2组中医证候积分均较治疗前明显降低(P均<0.01),且治疗组低于对照组(P<0.01);治疗后治疗组糜烂、胆汁反流、溃疡、出血斑改善程度均优于对照组(P<0.05或P<0.01);治疗后治疗组肠上皮化生和慢性炎症病理积分均较治疗前显著下降(P<0.05或P<0.01),且慢性炎症病理积分明显低于对照组(P<0.05);治疗后治疗组全血黏度高切、全血黏度低切、血浆黏度均低于对照组(P<0.05或P<0.01)。结论调和化瘀解毒法治疗慢性萎缩性胃炎临床疗效肯定,能明显改善患者临床症状和逆转病理组织变化,降低血黏度,改善瘀血状态,对临床有实用价值。Objective It is to observe the clinical effect of applying method of harmonizing liver and spleen,relieving of blood stasis and toxicity on chronic atrophicgastritis. Methods 67 patients with chronic atrophicgastritis were randomly divided into two groups： treatment group with 35 cases and control group with 32 cases. The treatment group was treated with Chinese medicine of invigorating spleen,regulating liver,relieving of blood stasis and toxicity. The control group was treated with Weifuchun tablet and Weimeisu tablet. All patients were treated for 3 months. The improvements of clinical symptoms,gastroscope image,pathology,and hemorheology in both groups before and after treatment were observed. Results The total effective rate of traditional Chinese medical syndrome in treatment group was significantly higher than that in the control group（ P〈0. 05）; The scores of traditional Chinese medical syndrome after treatment were decreased much compared with that before treatment in both groups（ P〈0. 01）,and the decrease was more significant in the treatment group（ P〈0. 01）; The improvement of gastroscope image in study group,such as erosion,bile regurgitation and petechiae,was superior to the control group after treatment,there was significant differences between the two groups（ P〈0. 05 or P〈0. 01）. The integral of intestinal metaplasia and chronic inflammation of stomach pathology in treatment group were decreased much compared with that before treatment（ P〈0. 05 or P〈0. 01）,and chronic inflammation scores of stomach pathology were lower than that in the control group（ P〈0. 05）; The improvement of hemorheology such as high shear rate,low shear rate and and plasma viscosity in treatment group was superior to the control group after treatment（ P〈0. 05 or P〈0. 01）. Conclusion The method of harmonizing liver and spleen,relieving of blood stasis and toxicity has a good clinical curative effect on chronic atrophicgastritis,which can improve symptoms and the change of gastro
