CRP、PCT、IL-6对评价肝硬化伴全身炎性反应综合征预后的意义

摘　　要：目的了解肝硬化伴全身炎性反应综合征(SIRS)患者C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素6(IL-6)水平,探讨各指标预测预后的能力。方法收集肝硬化伴SIRS患者56例,依预后分为好转组39例,死亡组17例,出现SIRS后第1、3、7天检测CRP、PCT、IL-6水平,观察CRP、PCT、IL-6与预后的关系。结果第1天两组比较CRP、PCT、IL-6差异均无统计学意义(P>0.05);第3天两组相比较PCT、IL-6差异无统计学意义(P>0.05),好转组CRP明显下降(P<0.05);第7天两组CRP、PCT、IL-6比较,好转组均明显低于死亡组(P<0.05)。同组相比,死亡组CRP、PCT、IL-6第1天、第3天、第7天检测值差异无统计学意义(P>0.05);好转组PCT、IL-6第3天与第1天相比差异无统计学意义(P>0.05),CRP第3天、第7天明显下降(P<0.05),好转组PCT、IL-6第7天与第1天相比显著下降(P<0.05)。结论 CRP、PCT、IL-6的持续高水平提示预后不良;与PCT、IL-6相比,CRP更能迅速反应病情改善。Objective. To detect the serum levels of C-reactive protein （CRP）,proealeitonin （PCT）,and interleukin-6（IL-6） and evaluate their prognostic values in liver cirrhosis with systemic inflammatory response syndrome（SIRS）. Methods A total of 56 patients with liver cirrhosis and systemic inflammatory response syndrome were divided into two groups, improved group（n = 39） and death group（n= 17）. Serum CRP,PCT,IL-6 levels were measured at the first day, third day and seventh day after SIRS. Results CRP,PCT and IL-6 levels of two groups were no statistically significant difference in the first day （P〈0.05）. PCT and IL-6 levels of two groups were no statistically significant difference in the third day （P〈0.05）. CRP levels in improved group were lower than those in death group in the third day （P〈0.05）. CRP,PCT and IL-6 levels in improved group were lower than those in death group in the seventh day （P〈0.05）. There was no statistically significant difference of CRP,PCT and IL-6 at the first day, third day and seventh day in death group （P〈0.05）. CRP levels at the third day and seventh day were lower than those at the first day in improved group （P〈0.05）. PCT and IL-6 levels at the seventh day were lower than those at the first day in improved group （P〈0.05）. Conclusion Poor prognosis was showed at sustained high levels of CRP,PCT and IL-6. Compared with PCT and IL-6, CRP might be more rapid in showing condition improved.
