超乳手术治疗白内障合并老年痴呆的应用研究

摘　　要：目的:评价超乳手术治疗白内障合并轻中度老年痴呆的临床应用价值。方法:年龄相关性白内障合并轻中度老年痴呆患者30例(30眼),均行超声乳化白内障摘除联合后房折叠型人工晶体植入术,所有患者随访6个月,评估患者手术前及术后6个月裸眼视力、认知状况、日常生活能力、精神症状及照料者的压力。结果:与手术前相比,患者术后6个月时的视力、Barthel指数得分明显升高,患者神经精神症状和照料者压力得分明显降低(P<0.01);术前、术后简易智能量表评分比较,差异无统计学意义(P>0.05)。结论:超声乳化手术可改善白内障合并老年痴呆患者的视功能和日常生活能力,维持患者的认知状态,减轻患者的神经精神症状和照料者的压力。Objective： To evaluate the clinical value of phacoemulsification for treating cataract patients combined senile dementia. Methods： 30 age-related cataract patients（30 eyes） combined with mild to moderate senile dementia were treated with phacoemulsification and implantation of Rayner foldable posterior chamber intraocular lens. All the patients were followed up for 6 months. The uncorrected visual acuity, cognitive condition, activity of daily living, mental symptom and caregiver pressure of patients were respectively evaluated before operation and 6 months after operation. Results： Compared with that before operation, the uncorrected visual acuity, Barthel index increased obviously, mental symptom score and caregiver pressure score decreased obviously（P 〈0.01）. There had no statistical significance about MMSE score before and after operation（P 〈0.05）. Conclusions： Phacoemulsification can improve the visual acuity and activity of daily living, maintain cognitive state, reduce the neuropsychic symptoms and caregiver pressure of cataract patients combined senile dementia.
