摘 要:目的探讨角膜基质内羊膜(amniotic membrane,AM)移植治疗兔大泡性角膜病变(bullous keratopathy,BK)的疗效。方法 40只新西兰健康成年白兔,建立BK模型,然后随机分为角膜基质内AM移植组(A组,12只)、角膜表面AM移植组(B组,12只)、切开角膜板层组(C组,12只)、对照组(D组,4只),比较四组兔移植后不同时间的中央角膜厚度和角膜中央混浊情况。结果 AM移植前四组兔的中央角膜厚度差异无统计学意义(F=0.184,P=0.90),而在移植后第1、2、4、8、12周四组兔中央角膜厚度组间差异均有显著统计学意义(均为P<0.01)。AM移植后4周时,A组兔的角膜中央混浊情况为+者7眼、++者1眼,B、C组均为+者2眼、++者6眼,D组为+者3眼、++者5眼,组间差异有统计学意义(χ2=13.21,P<0.01),而其他时间点差异均无统计学意义(均为P>0.05)。结论角膜基质内AM移植可以有效修复BK兔角膜内皮细胞。Objective To discuss the curative effects of amniotic membrane (AM) transplantation in corneal matrix for bullous keratopathy (BK). Methods BK models in forty healthy adult white New Zealand rabbits were established and divided into group A (AM transplantation in corneal matrix), group B (AM transplantation on corneal epidermis), group C ( corneal incision) and group D ( control group) according to management options at random. The central corneal thickness and corneal opacification status at different time after transplantation in four groups were compared. Re- sults There was no statistical difference in central corneal thickness before transplantation among four groups (F = 0. 184, P = 0.90 ). There were significant statistical differences in central corneal thickness at 1 week, 2 weeks, 4 weeks, 8 weeks and 12 weeks after transplantation among four groups ( all P 〈 0.01 ). At 4 weeks after trans- plantation, the central corneal opacification status ( + ) was in 7 eyes and ( + + ) in I eye of group A,2 eyes and 6 eyes of group B and C,3 eyes and 5 eyes of group D, there was significant difference among four groups (X2 = 13.21 ,P 〈 0.01 ), but there was no statistical difference at other time points ( all P 〉 0.05 ). Conclusion AM transplan- tation in corneal matrix can repair the corneal endothelial cells in rabbits with BK.
