多发性骨髓瘤患者血清铁调素与白细胞介素-6表达水平及其与贫血的关系

摘　　要：目的探讨多发性骨髓瘤（MM）患者血清铁调素及白细胞介素（IL）-6表达水平及与贫血的相关性关系。方法选取江苏省太仓市第一人民医院血液肿瘤科于2010年1月至2014年12月收治的32例MM患者为研究对象，并纳入研究组。选取同期于本院体检的健康个体20例纳入对照组。本研究遵循的程序符合江苏省太仓市第一人民医院人体试验委员会所制定的伦理学标准，得到该委员会批准，征得受试对象的知情同意，并与之签署临床研究知情同意书。应用酶联免疫吸附试验（ELISA）方法检测两组受试者血清铁调素与IL-6水平，应用XE2100全自动血液分析仪检测两组受试者血红蛋白（Hb）水平，应用化学发光法检测两组受试者铁蛋白水平，并分析MM患者血清铁调素水平与IL-6、铁蛋白及Hb水平相关性关系。结果①多发性骨髓瘤患者的铁调素、IL-6及铁蛋白的水平分别为（6．06±4．78）μg／L、（35．23±31．37）ng／L和（351．52±256．93）μg／L，均显著高于对照组，且差异均有统计学意义（P〈0．05）。②铁调素水平与IL-6、铁蛋白浓度水平呈正相关关系（r=0．545，0．593；P=0．007，0．003），铁调素水平则与Hb水平呈负相关关系（r=-0．494，P=0．017）。结论MM患者血清铁调素及IL-6表达水平均增高，铁调素及IL-6可能参与患者慢性病贫血的发生。Objective To investigate the expression levels of hepcidin, interleukin （IL）-6 and their relation with anemia in multiple myeloma （MM） patients. Methods From January 2010 to December 2014, a total of 32 MM patients who were diagnosed as MM in the First People＇ s Hospital of Taicang were collected into this study, as study group （n=32）. And 20 cases of healthy person in the same period were collected into control group. The study protocol was approved by the Ethical Review Board of Investigation in Human at the First People＇s Hospital of Taicang. Informed consent was obtained from all participants. The serum levels of hepcidin and IL-6 were tested by enzyme-linked immuosorbent assay （ELISA）. The levels of hemoglobin （Hb） were detected by XE2100 fully automatic hematology analyzer. The serum ferritin were measured by chemiluminescence method in two groups. The relation between hepcidin, IL-6 and Hb was analyzed. Results （1）The serum levels of hepcidin, IL-6 and serum ferritin in MM patients were （6.06±4. 78） gg/L, （35.23±31.37） ng/L and （351. 52±256.93）μg/L, which were significantly higher than those of control group （P〈0.05）. （2）The expression level of hepcidin was positively correlated with serum ferritin and IL-6 levels （r=0. 545,0. 593;P=0. 007, 0. 003） , and it was negatively correlated with Hb level （r= -0. 494,P=0. 017）. Conclusions The expression levels of hepcidin and IL-6 in MM patients were increased and may play an etiologic role in anemia of chronic disease.
